摘　　要：目的探讨椎间混合植骨联合单边椎弓根内固定MIS-TLIF手术治疗腰椎退行性疾病的临床疗效。方法采用椎间混合植骨联合单边椎弓根内固定MIS-TLIF手术治疗40例腰椎退行性疾病患者,观察手术时间、术中出血量、椎间植骨融合率。采用疼痛VAS评分和JOA评分评定疗效。分析评价术前、术后患者的VAS评分、腰椎JOA评分以及椎间融合率。结果手术时间60~100 min,术中失血量150~250 mL。所有患者均获得随访,时间为3个月。术前VAS评分6~8(7.1±0.8)分,末次随访2~4(2.8±0.7)分,差异有统计学意义(P<0.05)。术前JOA评分9~14(11.±2.4)分,末次随访16~28(21.9±6.2)分,差异有统计学意义(P<0.05)。结论采用椎间混合植骨联合单边椎弓根内固定MIS-TLIF手术治疗腰椎退行性疾病,疗效满意,具有手术时间短、创伤小、椎间融合率高等优点;在自体骨量不足的前提下,混合植骨不失为一种理想的植骨方式。Objective To investigate the clinical efficacy of mixed intervertebral bone grafting combined with unilateral arch root internal fixation MIS-TLIF surgery in the treatment of lumbar degenerative diseases.Methods Forty patients with lumbar degenerative diseases were treated with mixed intervertebral bone grafting combined with unilateral arch root internal fixation MIS-TLIF surgery,and the operative time,intraoperative bleeding and intervertebral bone grafting fusion rate were observed.Pain VAS score and JOA score were used to evaluate the efficacy.The VAS score,lumbar JOA score,and interbody fusion rate of the patients before and after operation were analyzed.Results The operative time ranged from 60 to 100 min,and intraoperative blood loss ranged from 150 to 250 mL.All patients were followed up for 3 months.Preoperative VAS score ranged from 6 to 8(7.1±0.8),and at the last follow-up 2 to 4(2.8±0.7),with statistically significant difference(P<0.05).Preoperative JOA score ranged from 9 to 14(11±2.4)and 16 to 28(21.9±6.2)at the last follow-up,with statistically significant difference(P<0.05).Conclusion The use of mixed intervertebral bone grafting combined with unilateral arch root internal fixation MIS-TLIF surgery for the treatment of lumbar degenerative diseases has satisfactory efficacy,with the advantages of short operation time,small trauma,and high intervertebral fusion rate;under the premise of insufficient autologous bone volume,mixed bone grafting is a desirable bone grafting method.
