加味黄连阿胶汤改善心肾不交型中风后睡眠障碍的临床研究

摘　　要：目的观察加味黄连阿胶汤治疗心肾不交型中风后失眠的临床疗效。方法选取中风后失眠症患者60例,随机分为中药组和西药组各30例,中药组用加味黄连阿胶汤治疗,西药组用艾司唑仑治疗,4周后,比较两组临床疗效和中医证候积分,评价睡眠质量,并比较Flinders疲劳量表、日常生活能力评定评分。结果中药组总有效率(86.67%)高于西药组(66.67%),差异有统计学意义;治疗后,两组中医证候积分、PSQI指数、FFS评分、改良Barthel指数均明显改善,差异有统计学意义,中药组PSQI指数、FFS评分、中医证候积分低于西药组,改良Barthel指数评分显著高于西药组,组间差异有统计学意义。结论加味黄连阿胶汤对于心肾不交型中风后睡眠障碍的治疗疗效确切,可提高患者睡眠质量,解除疲劳,提高日常生活能力。Objective To observe the clinical effect of modified Huanglian Ejiao decoction on insomnia after stroke of heart kidney disharmony type.Methods Sixty patients with post-stroke insomnia were randomly divided into the Chinese medicine group and the western medicine control group,with 30 patients in each group.The traditional Chinese medicine group was treated with modified Huanglian Ejiao Decoction,while the western medicine group was treated with estazolam.After 4 weeks,the clinical efficacy and TCM syndrome scores of the two groups were compared,the sleep quality was evaluated,and the Flinders Fatigue Scale(FFS)and Daily Living Ability Assessment(Modified Barthel Index)were compared.Results The total effective rate of Chinese medicine group(86.67%)was higher than that of western medicine group(66.67%),and the difference was statistically significant.After treatment,TCM syndrome score,PSQI index,FFS score and improved Barthel index were significantly improved in both groups,and the difference was statistically significant.PSQI index,FFS score and improved Barthel index score in traditional Chinese medicine group were lower than those in the western medicine group,while the improved Barthel index score was significantly higher than that in the western medicine group,and the difference between groups was statistically significant.Conclusion Modified Huanglian Ejiao Decoction has a definite therapeutic effect on sleep disorder after stroke of heart kidney disharmony type,which can improve the sleep quality of patients,relieve fatigue and improve the ability of daily life.
