摘 要:目的 比较平衡式稳态自由进动磁共振血管成像(b-SSFP-MRA)和对比增强磁共振血管成像(CE-MRA)在老年2型糖尿病肾病(T2DKD)肾动脉成像的图像质量、分支数显影情况,探讨b-SSFP序列在老年T2DKD肾动脉成像的应用价值。方法 收集2012年1月—2022年11月在复旦大学附属华东医院经临床诊断为T2DKD行肾动脉MRA的老年(年龄≥60岁)受检者165例,根据受检技术不同,分为b-SSFP-MRA组(105例)和CE-MRA组(60例),再根据病情将每组受检者划分为轻中度和重度。分别比较轻中度和重度组的两种检查方法的肾动脉图像质量、分支数显影情况。结果 (1)轻中度组:b-SSFP-MRA组和CE-MRA组的肾动脉图像质量、分支数显影情况差异均无统计学意义(P>0.05);(2)重度组:b-SSFP-MRA组和CE-MRA组的肾动脉图像质量差异无统计学意义(P>0.05),二者的分支数显影情况差异有统计学意义(P<0.05),b-SSFP-MRA能提供较好的肾动脉远端分支图像。结论 磁共振b-SSFP序列是一种成像速度快、安全性更高、成像质量较好的扫描技术,对评估老年T2DKD肾动脉受累情况是一种较好的无创选择方式。Objective To compare the image quality and number of branches developed of balanced steady⁃state free precession magnetic resonance angiography(b⁃SSFP⁃MRA)and contrast⁃enhanced magnetic resonance angiography(CE⁃MRA)in renal artery imaging of elderly patients with type 2 diabetes nephropathy(T2DKD),and explore the application val⁃ue of b⁃SSFP sequence in renal artery imaging of elderly T2DKD patients.Methods 165 elderly T2DKD patients(aged≥60 years)who received renal artery MRA in Huadong Hospital Affiliated to Fudan University from 2012 to 2022 were collected.According to different examination techniques,they were divided into b⁃SSFP⁃MRA group(105 cases)and CE⁃MRA group(60 cases),and then the patients in each group were further divided into mild⁃moderate subjects and severe subjects according to their condition of disease.The image quality and number of branches developed of renal arteries of the two examination tech⁃niques were compared between the mild⁃moderate subjects and severe subjects.Results(1)In mild⁃moderate patients:There was not statistically significant difference in renal artery image quality and number of branches developed between the b⁃SSFP⁃MRA group and the CE⁃MRA group(P>0.05).(2)In severe subjects:There was no significant difference in renal artery image quality between the b⁃SSFP⁃MRA group and the CE⁃MRA group(P>0.05),but there was significantly different in number of branches developed between the b⁃SSFP⁃MRA group and the CE⁃MRA group(P<0.05).B⁃SSFP⁃MRA could pro⁃vide better images of distal branches of renal arteries.Conclusion Magnetic resonance b⁃SSFP sequence is a renal artery scan⁃ning technique with fast imaging speed,higher safety and better imaging quality,and it is a better non⁃invasive method for the assessment of renal artery involvement in elderly patients with T2DKD.
