摘　　要：目的 观察米力农注射液治疗老年肺心病合并Ⅱ型呼吸衰竭疗效及对患者血清α-羟丁酸脱氢酶(α-HBDH)、肌酸激酶(CK)水平的影响。方法 选取2019年2月-2022年2月成都市第三人民医院收治的肺心病合并Ⅱ型呼吸衰竭的116例老年患者为研究对象,按照数字表法随机分为对照组(n=58)和观察组(n=58)。对照组给予常规治疗,观察组在对照组治疗的基础上给予米力农注射液。观察并比较2组临床疗效、血清α-HBDH和CK水平、血气指标值[氧合指数、二氧化碳分压(PaCO_(2))、酸碱值(pH)]、血液流变学及不良反应。结果 观察组治疗有效率高于对照组(94.83%vs 72.41%)(P<0.05)。与同组治疗前比较,治疗后2组氧合指数、 pH均升高,α-HBDH、 CK、 PaCO_(2)及血液流变学检测值均降低(P<0.05),且观察组氧合指数高于对照组,而α-HBDH、 CK、 PaCO_(2)及血液流变学检测值低于对照组(P<0.05),但2组pH差异无统计学意义(P>0.05)。2组不良反应发生率差异无统计学意义(P>0.05)。结论 米力农注射液治疗老年肺心病合并Ⅱ型呼吸衰竭的疗效显著,可改善血气指标及血液流变学,降低α-HBDH、 CK水平,且安全性较高,具有一定的临床应用价值。Objective To observe the therapeutic effect of milrinone injection on elderly patients with cor pulmonale complicated with typeⅡrespiratory failure and its effect on the levels of serumα⁃hydroxybutyrate dehydrogenase(α⁃HBDH)and creatine kinase(CK).Methods A total of 116 elderly patients with cor pulmonale complicated with typeⅡrespiratory failure admitted to Third People􀆳s Hospital of Chengdu City from February 2019 to February 2022 were selected as the study subjects,and were randomly divided into control group(n=58)and observation group(n=58)according to the number table method.The control group received routine treatment,while the observation group received milrinone injection on the basis of the control group.The clinical efficacy,levels of serumα⁃HBDH and CK,blood gas indexes[oxygenation index,partial pres⁃sure of carbon dioxide(PaCO_(2)),pH value],hemorheological indexes and adverse reactions of the two groups were observed and compared.Results The effective rate of treatment in the observation group was higher than that in the control group(94.83%vs 72.41%)(P<0.05).Compared with the same group before treatment,the oxygenation index and pH value of both groups increased after treatment,and levels ofα⁃HBDH,CK,PaCO_(2) and hemorheological indexes decreased(P<0.05).The oxygenation index of the observation group after treatment was higher than that of the control group,while the levels ofα⁃HBDH,CK,PaCO_(2) and hemorheological indexes were lower than those of the control group(P<0.05),but there was no significant difference in pH value between the two groups after treatment(P>0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Milrinone injection has a significant effect on elderly patients with cor pulmonale complicated with typeⅡrespiratory failure.It can improve blood gas indexes and hemorheology,and reduce the levels ofα⁃HBDH and CK.Its safety is high and it has certain clinical application value.
