摘　　要：目的 探究ALDH 2基因多态性与老年2型糖尿病(T2DM)患者冠心病易感性及心血管事件发生关系。方法 选择2019年1月—2021年7月西部战区总医院就诊的226例老年T2DM患者纳入研究,分为冠心病组104例与非冠心病组122例;另选取同期年龄和性别匹配的150名体检健康者为对照组。采用RT-PCR法检测各组观察对象血白细胞中乙醛脱氢酶2(ALDH2)基因型。探究ALDH2基因型与冠心病易感性及心血管事件发生关系。结果 冠心病组A等位基因频率30.77%、 AA基因型频率19.23%,较非冠心病组的22.05%、 6.56%及对照组的17.00%、 6.00%均明显升高(P<0.05)。Logistic回归结果显示,携带ALDH 2突变型基因(AA/GA型)的T2DM患者发生冠心病的危险性较野生型(GG型)增加4.024倍(OR=4.024, 95%CI:1.157~19.425,P=0.018)。随访1年内,在冠心病组、非冠心病组中,携带ALDH2 AA型患者心血管事件发生率均明显高于GG型、 GA型(P<0.05)。结论 ALDH2的A等位基因可能与老年T2DM患者冠心病易感性有关,携带ALDH2 AA型基因可能增加T2DM患者心血管事件发生风险。Objective To explore the relationship between gene polymorphism of aldehyde dehydrogenase 2(ALDH2)and coronary heart disease susceptibility and cardiovascular events in elderly patients with type 2 diabetes mellitus(T2DM).Methods 226 elderly T2DM patients admitted to General Hospital of Western Theater Command from January 2019 to July 2021 were included and divided into coronary heart disease group(104 cases)and non⁃coronary heart disease group(122 ca⁃ses).Another 150 healthy individuals who were matched in age and gender during the same period were selected as the control group.RT⁃PCR was used to detect ALDH2 genotype in the white blood cells of the subjects.The relationship between ALDH2 genotype and susceptibility to coronary heart disease and occurrence of cardiovascular events were explored.Results The fre⁃quencies of A allele frequency and AA genotype in the coronary heart disease group were 30.77%and 19.23%,respectively,which were significantly higher than those in the non⁃coronary heart disease group(22.05%,6.56%)and the control group(17.00%,6.00%)(P<0.05).Logistic regression showed that T2DM patients with ALDH 2 mutant gene(AA/GA type)had 4.024 times higher risk of coronary heart disease than those with wild type(GG type)(OR=4.024,95%CI:1.157⁃19.425,P=0.018).Within one year of follow⁃up,the incidence of cardiovascular events in patients with ALDH2 AA geno⁃type in both coronary heart disease group and non⁃coronary heart disease group was significantly higher than those with GG and GA genotypes(P<0.05).Conclusion The A allele of ALDH 2 may be associated with susceptibility to coronary heart dis⁃ease in elderly patients with T2DM,and ALDH 2 AA genotype may increase the risk of cardiovascular events in patients with T2DM.
