2型糖尿病患者中CA19-9水平及其临床意义研究

摘　　要：目的探讨T2DM患者CA19-9升高与血糖不稳的相关性,并对CA19-9升高与T2DM患者高胰腺癌发病率的临床意义进行总结。方法选取201例T2DM住院患者为糖尿病组,另选取108例健康体检者作为对照组,收集入组者血脂代谢指标、消化道肿瘤标志物指标、血糖、糖化血红蛋白、空腹C肽等指标,分析CA19-9升高与T2DM患者血脂、血糖、糖化血红蛋白等指标的相关性。结果糖尿病组FPG、HbA1c、TG、CA19-9、CA50显著高于对照组(P<0.05);糖尿病组中HbA1c、FPG、CA50升高是CA19-9水平升高的具有独立相关因素;以患者糖化血红蛋白水平为横坐标作图,随着糖化血红蛋白的升高,CA19-9及CA50逐渐升高,且各组间比较P<0.05。结论在本文研究中T2DM患者肿瘤标记物CA19-9的升高与空腹血糖、糖化血红蛋白的升高独立相关,临床医生应严格控制血糖,以降低CA19-9水平,进而降低T2DM患者发生胰腺癌的风险。Objective To explore the correlation between increased CA19-9 and glycemic instability in patients with T2DM,and to summarize the clinical significance of increased CA19-9 and the high incidence of pancreatic cancer in T2DM patients.Methods We selected 201 cases of hospitalized patients with type 2 diabetes for diabetes group and other 108 routine physical examination as control group,collected their indexes of lipid metabolism,digestive tract tumor markers,blood glucose,glycosylated hemoglobin,fasting C peptide indicators.Results 1.FPG,HbAlc,TG,CA19-9 and CA50 in diabetic group were significantly higher than those in control group(P<0.05).2.Increased HbAlc,FPG and CA50 in diabetic group was an independent correlation factor of the increaseed CA19-9 level.3.It took the patient's glycated hemoglobin level as the abscissa,CA19-9 and CA50 gradually increased with the increase of glycated hemoglobin,and the comparison between the groups was P<0.05.Conclusions In this study,the increase of tumor marker CA19-9 in T2DM patients is independendy related to the increase of fasting blood glucose and glycosylated hemoglobin.CEnicians should strictly control blood glucose to reduce the level of CA19-9 and thus reduce the risk of pancreatic cancer in T2DM patients.
