切开复位微型钢板内固定术与钢丝内固定治疗创伤性掌指关节周围骨折的临床效果比较

摘　　要：目的比较切开复位微型钢板内固定术与钢丝内固定治疗创伤性掌指关节周围骨折的临床效果。方法选取2019年6月至2021年5月本院收治的80例创伤性掌指关节周围骨折患者为研究对象,依据入院编号的奇偶性将其分为参照组与试验组,每组40例。参照组行传统钢丝内固定治疗,试验组行切开复位微型钢板内固定术治疗。比较两组患者的治疗效果。结果试验组骨折恢复优良率高于参照组,愈合时间短于参照组,手术时间长于参照组(P<0.05)。治疗后6个月,试验组的视觉疼痛模拟量表(VAS)、手部关节活动能力、日常生活能力(ADL)评分及手部屈伸功能均优于参照组,差异具有统计学意义(P<0.05)。治疗后2周,试验组的白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平均显著优于参照组,差异具有统计学意义(P<0.05)。试验组的术后并发症总发生率显著低于参照组,差异具有统计学意义(P<0.05)。结论切开复位微型钢板内固定手术治疗创伤性掌指关节周围骨折患者具备理想效果,可显著提升复位效果,具有重要应用价值。Objective To compare the clinical effects of open reduction and microplate internal fixation and steel wire internal fixation in the treatment of traumatic fractures around metacarpophalangeal joints.Methods Eighty patients with traumatic fractures around metacarpophalangeal joints admitted in our hospital from June 2019 to May 2021 were selected as the research objects.According to the parity of admission numbers,the patients were divided into reference group and experimental group,with 40 cases in each group.The reference group was treated with steel wire internal fixation,and the experimental group was treated with open reduction and microplate internal fixation.The therapeutic effects of the two groups were compared.Results The excellent and good rate of fracture recovery of the experimental group was higher than that of the reference group,the healing time was shorter than that of the reference group,and the operation time was longer than that of the reference group(P<0.05).At 6 months after treatment,Visual Analogue Scale(VAS),hand joint mobility,Activities of Daily Living(ADL)scores and hand flexion and extension function of the experimental group were better than those of the reference group,and the differences were statistically significant(P<0.05).At 2 weeks after treatment,the levels of interleukin-6(IL-6),interleukin-10(IL-10),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)in the experimental group were significantly better than those in the reference group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the experimental group was significantly lower than that in the reference group,and the difference was statistically significant(P<0.05).Conclusion The open reduction and microplate internal fixation in the treatment of traumatic fractures around the metacarpophalangeal joints has ideal results,it can significantly improve the reduction effect,and has important application value.
