摘　　要：目的:探讨5A护理模式在激素冲击治疗胸腰椎骨折伴脊髓损伤患者期间的应用效果。方法:选取2020年7月—2022年7月经本院收治的胸腰椎骨折伴有脊髓损伤患者82例作为研究对象,随机分为对照组和观察组,每组41例。所有患者均行激素冲击疗法及椎体后路椎管减压AF内固定术,在此期间,对照组予以常规护理模式,观察组在对照组基础上另施以5A护理模式,比较两组功能康复效果、心理状况及自我管理行为及预后。结果:观察组FMA-L、BBS、ASIA感觉及运动评分、ESCA评分、BI评分高于对照组,SAS、SDS评分低于对照组(P<0.05),观察组并发症发生率为2.44%,低于对照组的19.51%(P<0.05)。结论:胸腰椎骨折伴有脊髓损伤患者在激素冲击疗法中应用5A护理模式,可加快功能康复,改善负性心理状态,提升自我管理行为,其可获有良好预后。Objective:To investigate the application value of 5A nursing model in hormone shock therapy for thoracolumbar fracture with spinal cord injury.Methods:A total of 82 patients with thoracolumbar fracture complicated with spinal cord injury admitted to our hospital from July 2020 to July 2022 were randomly divided into control group and observation group,with 41 cases in each group.All patients underwent hormone shock therapy and posterior vertebral canal decompression and AF internal fixation.During this period,the control group was given routine nursing mode,and the observation group was additionally given 5A nursing mode on the basis of the control group.The functional rehabilitation effect,psychological status,self-management behavior and prognosis of the two groups were compared.Results:The scores of FMA-L,BBS,ASIA sensory and motor,ESCA and BI in the observation group were higher than those in the control group,and the scores of SAS and SDS were lower than those in the control group(P<0.05).The incidence of complications in the observation group was 2.44%,which was lower than 19.51%in the control group(P<0.05).Conclusion:The application of 5A nursing mode in patients with thoracolumbar fracture and spinal cord injury in hormone shock therapy can accelerate functional rehabilitation,improve negative psychological state,improve self-management behavior,and obtain good prognosis.
