摘 要:目的 探索不同体质指数(BMI)的糖耐量异常患者肠道菌群代谢产物短链脂肪酸(SCFAs)的差异,为探索糖耐量异常患者肠道菌群代谢和发病之间的关系,进一步为肠道菌群优化糖耐量异常治疗方案提供理论依据。方法 将糖耐量异常患者根据BMI分成两个组:糖耐量异常组(IGT组),包括:偏瘦组(BMI≤23.9 kg/m^(2))和偏胖组(BMI≥24kg/m^(2));对照组为血糖正常的健康志愿者。通过气相色谱-质谱分析,检测、分析不同体型IGT患者的TC、TG、FINS和2hINS水平以及粪便中SCFAs中乙酸、丙酸、丁酸的浓度和差异。结果 IGT组的偏瘦组和偏胖组TC、TG、FINS和2hINS均增高,以偏胖组更为明显。IGT组的偏瘦组和偏胖组肠道菌群代谢产物中乙酸、丙酸浓度增高,丁酸减少(P<0.05)。TC、TG、FINS和2hINS水平与IGT组的偏瘦组和偏胖组的餐后2 h血糖呈正相关。肠道菌群代谢产物丁酸与偏瘦组和偏胖组餐后2 h血糖呈负相关;BMI、肠道菌群代谢产物乙酸、丙酸均与偏胖组餐后2 h血糖呈正相关;肠道菌群代谢产物丙酸与偏瘦组餐后2 h血糖呈正相关。结论 在糖耐量异常阶段,肠道菌群代谢产物SCFAs已经发生了变化,其中丙酸的浓度较明显的升高,而且不同BMI的IGT患者的肠道菌群代谢产物SCFAs变化也存在差异,这些变化与血糖的改变具有相关性。Objective To explore the differences of short chain fatty acids(SCFAs)in intestinal flora metabolism in patients with impaired glucose tolerance with different body mass index,and to provide theoretical basis for exploring the relationship between intestinal flora metabolism and disease in patients with impaired glucose tolerance,and to further optimize the treatment of impaired glucose tolerance through intestinal flora.Methods The patients with impaired glucose tolerance were divided into two groups based on the body mass index(BMI):IGT group,including underweight group(BMI≤23.9 kg/m^(2))and overweight group(BMI≥24 kg/m^(2)).The control group included healthy volunteers with normal blood glucose.The TC,TG,FINS and 2hINS levels of IGT patients with different body types and the concentrations of acetic acid,propionic acid and butyric acid in SCFAs of feces were detected and analyzed by gas chromatography-mass spectrometry.Results The TC,TG,FINS and 2hINS increased in underweight group and overweight group,especially in overweight group.In IGT group,the concentrations of acetic acid and propionic acid in intestinal microflora metabolites of underweight and overweight groups increased,while butyric acid decreased(P<0.05).The levels of TC,TG,FINS and 2hINS were positively correlated with the 2 h postprandial blood glucose of the lean and the obese IGT groups.Butyric acid,a metabolite of intestinal flora,was negatively correlated with 2 h postprandial blood glucose in the lean and the obese groups.BMI,intestinal flora metabolites acetic acid and propionic acid were positively correlated with 2 h postprandial blood glucose in the overweight group.Propionic acid,a metabolite of intestinal flora,was positively correlated with 2 h postprandial blood glucose in the lean group.Conclusion In the stage of impaired glucose tolerance,the changes in the intestinal microbiota metabolites SCFAs appear,and the concentration of propionic acid significantly increases.In addition,the changes in the SCFAs of the IGT patients with
