彩色多普勒超声检测子宫动脉血流变化在卵巢子宫内膜异位囊肿诊断中的价值

摘　　要：目的:观察彩色多普勒超声检测子宫动脉血流变化在卵巢子宫内膜异位囊肿诊断中的价值。方法:选择2020年1月~2021年12月在本院被诊断为卵巢子宫内膜异位囊肿的45例患者作为观察对象,患者均择期进行手术治疗,观察患者手术治疗前后激素水平、子宫动脉血流参数值变化情况以及治疗后的妊娠情况。结果:手术治疗1个月后,患者孕酮及雌二醇水平与治疗前比较明显升高(P<0.05);手术治疗前,患者囊肿侧子宫RI、PI及S/D参数检测值均显著高于健侧子宫检测值(P<0.05);手术治疗1个月后,患者囊肿侧子宫RI、PI及S/D参数检测值明显降低(P<0.05),且与健侧检测值比较无明显差异(P>0.05);手术治疗前,患者健侧子宫RI<0.8者为77.87%,显著高于囊肿侧的20.00%(P<0.05);手术治疗1个月后,患者囊肿侧子宫RI<0.8者百分比与手术前比较明显升高,且与健侧百分比相比无明显差异(P>0.05);32例不孕患者中,6例患者RI≥0.8,术后6个月内妊娠率为16.67%;26例患者RI<0.8,术后6个月内妊娠率为88.46%,差异具体统计学意义(P<0.05)。结论:彩色多普勒超声检测能够有效评估卵巢子宫内膜异位囊肿患者子宫动脉血流变化情况,可以为患者的临床诊断、治疗及预后情况的判定和提高可靠的参考依据,同时为子宫内膜异位症所致的不孕患者的诊疗及妊娠情况提供依据。Objective:To observe the value of color Doppler ultrasound in the diagnosis of ovarian endometriosis cyst.Methods:45 patients diagnosed as ovarian endometriosis cyst in our hospital from January 2020 to December 2021 were selected as the observation objects,so all patients were selected for surgical treatment,and the changes of hormone levels,uterine artery blood flow parameters and pregnancy after treatment were observed before and after surgical treatment.Results:After 1 month of operation,the levels of progesterone and estradiol in patients were significantly higher than those before treatment(P<0.05);Before operation,RI,PI and S/D parameters of the cyst side uterus were significantly higher than those of the healthy side uterus(P<0.05);After one month of surgical treatment,the RI,PI and S/D parameters of the cyst side uterus were significantly decreased(P<0.05),and there was no significant difference compared with those of the healthy side(P>0.05);Before operation,RI<0.8 in the healthy side of the uterus accounted for 77.87%,significantly higher than 20.00%in the cyst side(P<0.05);After 1 month of surgical treatment,the proportion of RI<0.8 in the cyst side uterus was significantly higher than that before surgery,and there was no significant difference compared with that in the healthy side(P>0.05);Among 32 infertile patients,6 patients had RI≥0.8,and the pregnancy rate within 6 months after operation was 16.67%;The RI of 26 patients was<0.8,and the pregnancy rate within 6 months after operation was 88.46%,with specific statistical significance(P<0.05).Conclusion:Color Doppler ultrasound can effectively evaluate the changes of uterine artery blood flow in patients with ovarian endometriosis cyst,provide reliable reference for clinical diagnosis,treatment and prognosis of patients,and provide basis for diagnosis,treatment and pregnancy of infertile patients caused by endometriosis.
