SaCo可视喉罩在中耳炎手术患者应用评价

摘　　要：目的:评价SaCo可视喉罩在中耳炎手术患者气道管理的有效性与安全性。方法:选择2021年5月~2022年3月本院择期行中耳炎手术患者50例,男21例,女29例,年龄18~65岁,BMI18~25kg/m^(2),ASAⅠ-Ⅱ级。全麻诱导后,根据患者性别、体重选择合适的喉罩置入,行机械通气,分别测量记录头颈正中位和侧旋位13cmH_(2)O压力控制通气下的潮气量、口咽漏气压、套囊内压、纤维支气管镜下咽部解剖结构显露分级,记录术后是否咽痛、声嘶等并发症,评价SaCo可视喉罩其可视化功能在指导喉罩置入与调整中的意义。结果:50例患者均一次性置入喉罩;与头颈正中位相比,侧旋位后的潮气量、套囊内压、漏气压、纤支镜下显露分级无统计学意义;在纤支镜对位良好下,SaCo可视喉罩对咽部解剖结构显露欠佳;有6例出现术后咽痛,发生率为12%,VAS评分均≤3分。结论:SaCo可视喉罩可安全用于中耳炎手术患者的气道管理,但用于评估喉罩对位情况则效果欠佳。Objective:To evaluate the effectiveness of airway management with SaCo visual laryngeal mask airway in otitis media surgery.Methods:50 patients scheduled for otitis media surgery,21 males and 29 females,aged 18-65 years,BMI18~25kg/m^(2),ASA physical statusⅠ-Ⅱwere enrolled from May 2021 to March 2022.After induction,a appropriate size of LMA was selected according to patient’s gender and weight,and then the patients were ventilated mechanically.Tidal volume at 13cmH_(2)O,intra-cuff pressure,oropharyngeal leakage pressure,the exposure of pharynx structures viewed by fiber bronchoscopy were respectively recorded at the head position of frontal and lateral rotation.Related complications such as sore throat,hoarseness were analyzed.The significance of visualization in instructing the placement and adjustment of SaCo were evaluated.Results:50 patients were successfully placed with a SaCo at the first try.The tidal volume,intra-cuff pressure,oropharyngeal leakage pressure and the exposure of pharynx structures viewed by fiber bronchoscopy were no significant difference in both head position.The exposure of pharynx structures viewed by the visualization of SaCo were significantly worse then fiber bronchoscopy.The incidence of postoperative sore throat was 12%and VAS score was less than 3.Conclusion:SaCo can be safely and effectively applied for airway management in otitis media surgery,but it’s not effective to evaluate the position of SaCo placed.
