胸部薄层CT检查对有癌症家族史体检人员早期肺部肿瘤的筛查效果

摘　　要：目的:分析胸部薄层CT检查对有癌症家族史体检人员早期肺部肿瘤的筛查效果。方法:选取2020年8月~2022年8月本院治疗的72例有癌症家族史体检人员为研究对象,所有受检者均接受胸部薄层CT检查、胸部X射线检查,以病理组织和穿刺病理组织为金标准,分析胸部薄层CT检查效果。结果:胸部薄层CT检查共计检出17例肺部结节,肺部结节检出率为23.61%,检出肺癌13例,肺癌检出率18.06%,常规胸部X射线检查共计检出4例肺部结节,肺部结节检出率为5.56%,检出肺癌1例,肺癌检出率1.39%,胸部薄层CT检查肺部结节、肺癌检出率均明显高于常规胸部X射线(P值均<0.05);同时,在肺结节分型与定性中,胸部薄层CT检查的诊断准确率明显高于常规胸部X射线(P值均<0.05);通过对不同大小的肺部结节诊断显示,在5mm以内的肺部结节诊断中,胸部薄层CT检查的诊断准确率明显高于常规胸部X射线(P值均<0.05)。结论:胸部薄层CT检查能够较为准确地筛查有癌症家族史体检人员早期肺部肿瘤,筛查效果显著优于胸部X射线检查。Objective:To analyze the screening effect of thin slice CT examination of chest on early lung tumors in physical examiners with family history of cancer.Methods:From August 2020 to August 2022,72 medical examiners with family history of cancer treated in our hospital were selected as the research objects.All the examinees received chest thin slice CT examination and chest X-ray examination.The results of chest thin slice CT examination were analyzed based on the gold standard of pathological tissue and puncture pathological tissue.Results:A total of 17 cases of pulmonary nodules were detected by thin chest CT,the detection rate of pulmonary nodules was 23.61%,13 cases of lung cancer were detected,the detection rate of lung cancer was 18.06%,4 cases of pulmonary nodules were detected by conventional chest X-ray,the detection rate of pulmonary nodules was 5.56%,1 case of lung cancer was detected,the detection rate of lung cancer was 1.39%,the detection rate of pulmonary nodules and lung cancer by thin chest CT was significantly higher than that by conventional chest X-ray(P<0.05);At the same time,in the classification and characterization of pulmonary nodules,the diagnostic accuracy of thin chest CT was significantly higher than that of conventional chest X-ray(P<0.05);The diagnosis of pulmonary nodules of different sizes showed that the diagnostic accuracy of thin chest CT was significantly higher than that of conventional chest X-ray in the diagnosis of pulmonary nodules within 5mm(P<0.05).Conclusion:Thin slice CT examination of the chest can more accurately screen early lung tumors of physical examiners with family history of cancer,and the screening effect is significantly better than chest X-ray examination.
