前列腺电切镜联合钬激光治疗复杂膀胱结石的有效性和安全性分析

摘　　要：目的:分析前列腺电切镜联合钬激光治疗复杂膀胱结石的有效性和安全性。方法:回顾性分析2019年1月~2021年12月收治的复杂膀胱结石患者78例为此次试验的研究对象,根据手术方案不同分组为参照组(n=39)、研究组(n=39),分别开展输尿管肾镜下钬激光碎石治疗、前列腺电切镜联合钬激光治疗。组间对比其手术治疗情况、术后并发症发生率以及残余尿量、最大尿流率、疼痛、生活活动能力、血清炎症因子指标变化。结果:研究组患者手术时间、膀胱冲洗时间、住院时间短于参照组,术中出血量少于参照组,术后并发症发生率2.56%低于参照组的17.95%,术后3d时残余尿量、VAS评分低于参照组,最大尿流率、ADL评分高于参照组,各血清炎症因子指标(IL-4、IL-6、hs-CRP、TNF-α)水平低于参照组,P<0.05。结论:前列腺电切镜联合钬激光治疗复杂膀胱结石,对改善患者手术情况及术后残余尿量、最大尿流率、疼痛、生活活动能力、血清炎症因子指标水平有积极意义,同时术后并发症发生率低,有积极临床应用价值。Objective:To analyze the efficacy and safety of prostatectomy combined with holmium laser in the treatment of complex bladder stones.Methods:A retrospective analysis was made on 78 patients with complex bladder stones who were admitted from January 2019 to December 2021.They were divided into the reference group(n=39)and the research group(n=39)according to the different surgical schemes.They were treated with holmium laser lithotripsy under ureterorenoscope and prostatectomy combined with holmium laser.The surgical treatment,postoperative complication rate,residual urine volume,maximum urine flow rate,pain,activity of life and serum inflammatory factors were compared between the two groups.Results:The operation time,bladder flushing time and hospitalization time of the patients in the study group were shorter than those in the reference group,the intraoperative bleeding volume was less than that in the reference group,the postoperative complication rate was 2.56%lower than that in the reference group by 17.95%,the residual urine volume and VAS score were lower than that in the reference group at 3 days after operation,the maximum urine flow rate and ADL score were higher than that in the reference group,and the serum inflammatory factors(IL-4,IL-6,hs CRP,TNF-α).The level was lower than that of the reference group(P<0.05).Conclusion:Prostatectomy combined with holmium laser in the treatment of complex bladder stones has positive significance in improving the surgical condition,postoperative residual urine volume,maximum urine flow rate,pain,life activity ability,serum inflammatory factor index level,and low postoperative complication rate.It has positive clinical value.
