改良式逆行法与传统再植法修复拇指末节离断伤疗效分析比较

摘　　要：目的 探讨改良式逆行法与传统再植法修复拇指末节离断伤的疗效。方法 便利选择2017年2月-2021年5月广东省茂名市人民医院收治的68例拇指末节离断患者,随机分为两组,每组34例。对照组采用常规顺序再植,观察组采用改良逆行顺序再植。分析比较两组手术成活率、手指功能、满意度。结果 观察组手术时间短于对照组,手术成活率(94.12%)高于对照组(76.47%),差异有统计学意义(P<0.05);术后3个月,观察组手指功能优良率(91.18%)高于对照组(70.59%),差异有统计学意义(χ^(2)=4.221、4.660,P<0.05)。结论改良式逆行法在拇指末节断指再植中减少了肢体翻动、易于寻找静脉、易于显露术野、缩短了断指再植时间、减少了缺血再灌注损伤,术后手指存活率高,手指外观、功能恢复良好,获得了良好的治疗效果。Objective To discuss the therapeutic effect of modified retrograde method and traditional replantation method in the repair of distal amputation of thumb.Methods Sixty-eight patients with severed distal segment of thumb admitted to the Maoming People's Hospital,Guangdong Province from February 2017 to May 2021 were conveniently selected and randomly divided into two groups,with 34 patients in each group.The control group was replanted in conventional sequence,while the observation group was replanted in modified retrograde sequence.The survival rate,finger function and satisfaction of the two groups were analyzed and compared.Results The operative time of the observation group was shorter than that of the control group,and the operative survival rate(94.12%)was higher than that of the control group(76.47%),the difference was statistically significant(P<0.05).Three months after operation,the excellent and good rate of finger function in the observation group(91.18%)was higher than that in the control group(70.59%),and the difference was statistically significant(χ^(2)=4.221,4.660,P<0.05).Conclusion The modified retrograde method can reduce the limb turning,easy to find veins,easy to reveal the operative field,shorten the replantation time of the severed finger,reduce the ischemia reperfusion injury,the postoperative survival rate of the finger is high,the appearance and function of the finger recovery is good,and obtain a good therapeutic effect.
