膝关节镜下半月板成形术治疗青年半月板损伤的疗效分析

摘　　要：目的 分析膝关节镜下半月板成形术治疗青年半月板损伤的疗效。方法 回顾性选取东南大学医学院附属江阴医院2016年11月—2021年5月间收治的青年半月板损伤患者80例,分为常规组和研究组,每组40例。常规组接受半月板切除术治疗,研究组接受膝关节镜下半月板成形术治疗。比较两组治疗效果、手术成效指标、治疗前后疼痛程度评分(VAS)、膝关节功能评分(末次随访6~12个月评估)、治疗前后生存质量测定、术后改良美国特种外科医院膝关节(HSS)评分(末次随访6~12个月评估)。结果 治疗前,两组VAS评分、生存质量测定结果比较,差异无统计学意义(P>0.05)。治疗后,研究组膝关节功能评分高于常规组,差异有统计学意义(P<0.05)。研究组治疗效果(97.50%)高于常规组(77.50%),差异有统计学意义(χ^(2)=7.314,P<0.05)。研究组VAS评分、术后改良HSS评分均优于常规组、生存质量评分高于常规组,差异有统计学意义(P<0.05)。结论 膝关节镜下半月板成形术能够及时救治青年患者半月板损伤,保留和恢复其膝关节的功能,提高生存质量,利于减少疼痛感,建议临床普及。Objective To analyze the effect of arthroscopic meniscus arthroplasty on meniscus injury in young adults.Methods A total of 80 young patients with meniscus injury were admitted to Jiangyin Hospital Affiliated to Medical College of Southeast University from November 2016 to May 2021 were retrospectively selected and divided into routine group and study group,with 40 cases in each group.The subjects in the conventional group were treated with meniscus resection,and the subjects in the study group were treated with arthroscopic meniscus plasty.Comparing the therapeutic effect,surgical effect index,pain degree score(VAS)before and after treatment,knee function score(6-12 moths evaluation at the last follow-up),quality of life before and after treatment,and postoperative modified Hospital for Special Surgery Knee Score(HSS)score(6-12 moths evaluation at the last follow-up)between the two groups.Results Before treatment,there was no statistically significant difference in VAS score and quality of life between the two groups(P>0.05).After treatment,the knee joint function of the study group was higher than that of the conventional group,and the difference was statistically significant(P<0.05).The treatment effect of the study group(97.50%)was higher than that of the conventional group(77.50%),and the difference was statistically significant(χ^(2)=7.314,P<0.05).The VAS score and postoperative modified HSS score of the study group were better than those of the conventional group,and the quality of life score was higher than that of the conventional group,and the difference was statistically significant(P<0.05).Conclusion Arthroscopic meniscus arthroplasty can timely save meniscus injury in young patients,preserve and restore the function of the knee joint,improve the quality of life to reduce pain,so it is recommended to be popularized in clinic.
