晶状体摘出术对原发性闭角型青光眼并发白内障患者的疗效及并发症发生率分析

摘　　要：目的 讨论晶状体摘出术对原发性闭角型青光眼并发白内障患者的疗效及并发症发生率的影响。方法 随机选取2020年5月—2021年5月福建省立金山医院收治的原发性闭角型青光眼并发白内障患者50例为研究对象,将其随机分为对照组25例与研究组25例。对照组实施周边虹膜切开术,研究组实施晶状体摘出术。统计对比两组疗效,并发症发生率,手术前后前房深度、眼压、视力,以及术后所用恢复时间。结果研究组疗效总有效率96.00%明显高于对照组的68.00%,差异有统计学意义(χ^(2)=4.878,P<0.05)。研究组并发症发生率4.00%明显低于对照组的32.00%,差异有统计学意义(χ^(2)=4.878,P<0.05)。术前两组患者的前房深度、眼压及视力指标比较,差异无统计学意义(t=0.867、0.041、0.542,P>0.05);术后3个月,研究组前房深度(4.32±0.29)mm及视力(0.98±0.05)指标均明显高于对照组,眼压(13.30±2.28)mmHg明显低于对照组,差异有统计学意义(t=19.246、30.224、6.601,P<0.001)。研究组术后恢复时间(12.34±2.69)d明显短于对照组,差异无统计学意义(t=7.432,P<0.001)。结论 晶状体摘出术治疗原发性闭角型青光眼并发白内障病症的疗效明显高于周边虹膜切开术,且前者手术方案并发症更少,更利于术后恢复前房深度、眼压、视力水平,缩短术后康复时间。Objective To discuss the effect of lens extraction on the efficacy and complication rate of patients with primary angle closure glaucoma complicated with cataract.Methods Fifty patients with primary angle-closure glaucoma complicated by cataract admitted to Fujian Jinshan Hospital from May 2020 to May 2021 were randomly selected as the study objects,and were randomly divided into a control group of 25 patients and a study group of 25 patients.The control group underwent peripheral iridotomy and the study group underwent lens extraction.The efficacy,complication rate,anterior chamber depth,intraocular pressure,visual acuity before and after surgery,recovery time used after surgery were observed and compared.Results The total efficacy rate of 96.00% in the study group was significantly higher than 68.00% in the control group,the difference was statistically significant(χ^(2)=4.878,P<0.05).The complication rate of 4.00% in the study group was significantly lower than 32.00% in the control group,the differences was sta-tistically significant(χ^(2)=4.878,P<0.05).There were no statistically significant differences in the anterior chamber depth,intraocular pressure and visual acuity indexes between the two groups before surgery(t=0.867,0.041,0.542,P>0.05).At 3 months after surgery,the indexes of anterior chamber depth(4.32±0.29) mm and visual acuity(0.98±0.05)in the study group were significantly higher than those in the control group,and IOP(13.30±2.28) mmHg was significantly lower than that in the control group,and the difference was statistically significant(t=19.246,30.224,6.601,P<0.001).The postoperative recovery time index of the study group(12.34±2.69) d was significantly shorter than that of the control group,the differences was statistically significant(t=7.432,P<0.001).Conclusion Lens extraction was significantly more effective than peripheral iridotomy in the treatment of primary angle-closure glaucoma complicated by cataract disease,and the former surgical protocol had fewer complications and was more
