摘 要:目的 探讨静脉营养中加入甘油磷酸钠对极低出生体质量儿或极早产儿骨代谢的影响。方法 回顾性分析收治的116例出生体质量<1 500 g或孕周<32周的极早产儿的病历资料,按照静脉营养中是否加入甘油磷酸钠注射液分为观察组(53例)和对照组(63例)。2组患儿均补充钙和维生素D,观察组在出生后第2天静脉营养中即加入甘油磷酸钠注射液。2组均治疗2~3周。记录患儿母亲孕期一般情况、妊娠期合并症,患儿胎龄、性别和出生体质量等;比较极早产儿出生后1 d内及1、2、4、6周的血清钙离子、血清磷、血清碱性磷酸酶(alkaline phosphatase,ALP)水平。结果 2组患儿均未发现多汗、烦躁、易惊和枕秃等临床症状及体征。2组患儿及母亲一般资料比较差异无统计学意义(P>0.05)。出生后1、2、4、6周,2组血清钙离子水平均高于出生后1 d内,且出生后1、2、4、6周,观察组患儿血清钙离子水平均高于对照组(P<0.05)。出生后1、2、4、6周,2组血清磷水平均低于出生后1 d内。出生后1、2、4、6周,2组血清ALP水平均高于出生后1 d内,出生后2、4、6周高于出生后1周;对照组出生后6周ALP水平高于出生后2、4周,观察组出生后6周ALP水平高于出生后2周,低于出生后4周;出生后2、4、6周,观察组患儿ALP水平均低于对照组(P<0.05)。结论 静脉营养中早期加入甘油磷酸钠注射液补充磷元素可显著改善极早产儿的骨代谢指标,利于血清钙磷稳定和骨骼代谢状态。Objective To determine the effect of the addition of sodium glycerophosphate to intravenous nutrition on bone metabolism in very low birth weight or very preterm infants.Methods The medical records of 116 very preterm infants with a birthweight<1500 g or a gestational age<32 weeks were retrospectively reviewed.The cases were divided into observation group(53 cases)and control group(63 cases)according to whether or not Sodium Glycerophosphate Injections was added to the intravenous nutrition.The infants in both groups were given calcium and vitamin D supplementation,and in the observation group,sodium glycerophosphate injection was added to intravenous nutrition at 2nd day after birth.The both groups were treated for 2-3 weeks.The general conditions of mothers,comorbidities during pregnancy,and gestational age,sex,and birth weight of the affected child were recorded.The levels of serum calcium,serum phosphorus and serum alkaline phosphatase(ALP)of premature infants were collected at the 1st,2nd,4th and 6th weeks after birth were compared.Results No clinical signs and symptoms such as hyperhidrosis,irritability,easily startled and occipital baldness were found in either group.There was no significant difference in the general data of the children and mothers between the 2 groups(P>0.05).At the 1st,2nd,4th and 6th weeks after birth,the levels of serum calcium in the 2 groups was higher than that at the 1st day after birth,and at the 1st,2nd,4th and 6th weeks after birth,the children in the observation group were higher than those in the control group(P<0.05).At the 1st,2nd,4th and 6th weeks after birth,the levels of serum phosphorus in the 2 groups was lower than that at the 1st day after birth.At the 1st,2nd,4th and 6th weeks after birth,the levels of serum ALP in both groups were higher than those at the 1st day after birth,At the 2nd,4th and 6th weeks after birth were higher than those at the 1st week after birth.In the control group,the level of ALP at 6 weeks after birth was higher than that at the 2nd and 4th
