摘　　要：目的探讨T2DM患者细胞外液、细胞内液体积比值(ECW/ICW)与UACR的相关性。方法选取2021年5月至2022年4月于空军军医大学唐都医院内分泌科住院治疗的T2DM患者322例,根据UACR分为<30 mg/g的蛋白尿正常组(Con,n=175)、30≤UACR≤300 mg/g的微量蛋白尿组(Mic,n=78)和>300 mg/g的大量蛋白尿组(Mac,n=69)。比较各组一般资料及生化指标,Spearman相关分析T2DM患者ECW/ICW与UACR的相关性,二元Logistic回归分析T2DM患者UACR的影响因素。结果Mac组DM病程、SBP、DBP、TC、LDL-C、BUN、血肌酐、血尿酸、ECW/ICW、内脏脂肪面积(VFA)高于Con、Mic组(P<0.05或P<0.01),eGFR低于Con、Mic组(P<0.05)。Spearman相关分析显示,ECW/ICW与UACR呈正相关(r=0.157,P=0.005)。二元Logistic回归分析显示,ECW/ICW是T2DM患者UACR的影响因素。结论ECW/ICW是T2DM患者UACR的影响因素,对监测DKD进展及随访有一定临床价值。Objective To explore the correlation between the volume ratio of extracellular fluid and intracellular fluid(ECW/ICW)and urinary albumin creatinine ratio(UACR)in type 2 diabetes mellitus(T2DM)patients.Methods 322 T2DM patients hospitalized in the Department of Endocrinology,Tangdu Hospital,Air Force Military Medical University from May 2021 to April 2022 were divided into normal proteinuria group(Con,UACR<30 mg/g,n=175),microalbuminuria group(Mic,30≤UACR≤300 mg/g,n=78)and massive proteinuria group(Mac,UACR>300 mg/g,n=69)according to the level of UACR.The general data and biochemical indicators of each group were compared.The correlation between ECW/ICW and UACR and the influencing factors of UACR in T2DM patients were analysed.Results The DM duration,SBP,DBP,TC,LDL-C,BUN,serum creatinine,serum uric acid,ECW/ICW and visceral fat area(VFA)in Mac group were higher,and eGFR was lower than those in Con and Mic group(P<0.05 or P<0.01).Spearman correlation analysis showed that ECW/ICW was positively correlated with UACR(r=0.157,P=0.005).Binary logistic regression analysis showed that ECW/ICW was the influencing factor of UACR in T2DM patients.Conclusion ECW/ICW is an independent influencing factor of UACR in T2DM patients,and has certain value in monitoring the development and follow-up of diabetic nephropathy.
