重型脑挫裂伤术后昏迷患者主要照顾者负担 睡眠质量与社会支持的相关性

摘　　要：目的探讨重型脑挫裂伤标准大骨瓣减压术后昏迷患者主要照顾者负担、睡眠质量与社会支持的关系。方法对96名重型脑挫裂伤标准大骨瓣减压术后昏迷患者主要照顾者的照顾负担、睡眠质量及社会支持状况进行调查,对主要照顾者的照顾负担、睡眠质量与社会支持状况的关系采用Pearson相关分析。结果入组照顾者中照顾负担量表评分为(56.34±5.27)分,负担程度重度占54.04%,中度占29.17%,轻度占15.63%,极轻度或无占4.16%。入组照顾者匹兹堡睡眠质量指数评分为(7.28±0.75)分,睡眠质量良好占8.33%,中等占28.13%,较差占63.54%。入组照顾者社会支持评定量表评分为(24.36±4.96)分,社会支持获得低水平占57.29%,中等水平占29.17%,高水平占13.54%。Pearson相关分析显示,入组照顾者社会支持评定量表评分与照顾负担量表评分、匹兹堡睡眠质量指数评分呈显著负相关(P<0.05)。结论重型脑挫裂伤标准大骨瓣减压术后昏迷患者主要照顾者社会支持度越低,照顾负担重,睡眠质量差,通过提升其社会支持可改善其照顾负担和睡眠质量。Objective To explore the relationships among burden,sleep quality,and social support in primary caregivers of comatose patients after standard decompressive craniectomy(SDC)for heavy brain contusion and laceration(BCL).Methods Investigations of caregiving burden,sleep quality and social support status were conducted in 96 primary caregivers of comatose patients following SDC for severe BCL,Pearson correlation analyses used to the relationships among burden,sleep quality,and social support.Results Score on the Zarit Burden Interview(ZBI)of enrolled caregivers was(56.34±5.27),severe burden made up 54.04%,moderate 29.17%,mild 15.63%,and very mild or none 4.16%.Scores of enrolled caregivers on Pittsburgh Sleep Quality Index(PSQI)were(7.28±0.75),good sleep made up 8.33%,moderate 28.13%for and poor 63.54%.Scores of enrolled caregivers on the Social Support Rating Scale(SSRS)were(24.36±4.96),low social support made up 57.29%,moderate 29.17%,and high 13.54%.Pearson correlation analysis showed that SSRS scores of enrolled caregivers were negatively correlated with ZBI and PSQI scores significantly(P<0.05).Conclusion The lower the social support of primary caregivers of comatose patients after SDC for severe BCL,the heavier the caregiving burden and the poorer the sleep quality,and caregiving burden and sleep quality could be improved via enhancing social support.
