摘 要:目的 分析2019冠状病毒病(COVID-19)疫情期间我院的儿科门诊呼吸道感染就诊量变化及其特点。方法 回顾性分析2019年1月1日至2021年12月31日吴川市妇女儿童医院儿科门诊收治的125 632例0~16岁患儿的病例资料,观察儿科门诊呼吸道感染患儿的发病年龄、季节分布情况。结果 2019—2021年我院儿科门诊量均小幅度增长;2019年、2020年、2021年上呼吸道感染门诊就诊量明显高于肺炎和肠炎,差异均有统计学意义(P<0.05);2019年上呼吸道感染就诊量为24 189例,明显高于2020年23 723例和2021年21 820例,差异均有统计学意义(P<0.05);2019—2021年各年份间呼吸道感染就诊量中的性别和年龄构成比总体比较差异无统计学意义(P>0.05),其中男性患儿占比较高,婴幼儿占比最高(分别为63.36%、59.32%、68.18%),其次为学龄前儿童(分别为28.27%、30.04%、28.96%)和学龄期儿童(分别为8.36%、10.59%、14.29%);2019年、2020年内的前三季度就诊量比较差异无统计学意义(P>0.05),第四季度2021年就诊量最高,其次为2019年、2020年,差异有统计学意义(P<0.05)。结论 COVID-19疫情前后儿科门诊呼吸道感染就诊量的占比构成和顺位变化较小,2020~2021年时期内儿童患呼吸道感染性疾病的数量明显降低。Objective To analyze the changes and characteristics of the number of respiratory tract infection visits in Pediatric Out-patient Department during the 2019 coronavirus disease(COVID-19)epidemic.Methods The clinical data of 125632 children aged 0-16 who were admitted to the Pediatric Out-patient Department of Wuchuan Women's and Children's Hospital from January 1,2019 to December 31,2021 were retrospectively analyzed.The age and seasonal distribution of respiratory tract infection were observed.Results From 2019 to 2021,the number of pediatric outpatients increased slightly.The number of upper respiratory tract infections in 2019,2020,and 2021 was significantly higher than that of pneumonia and enteritis(P<0.05).The number of patients with upper respiratory tract infection in 2019 was 24189,which was significantly higher than 23723 in 2020 and 21820 in 2021(P<0.05).There was no statistically significant difference in gender and age composition ratio of respiratory tract infection visits among 2019,2020,and 2021(P>0.05).Among the patients,male children accounted for a higher proportion,with infants accounted for the highest proportion(63.36%,59.32%,and 68.18%),followed by preschool children(28.27%,30.04%,28.96%),and then school-age children(8.36%,10.59%and 14.29%).There was no significant difference in the number of visits in the first three quarters of 2019 and 2020(P>0.05),and the number of visits in the fourth quarter was the highest in 2021,followed by 2019 and 2020,with a statistically significant difference(P<0.05).Conclusion Before and after the COVID-19 epidemic,the proportion and order of the number of respiratory tract infection visits in Pediatric Out-patient Department changed slightly,and the number of children suffering from respiratory infectious diseases decreased significantly from 2020 to 2021.
