摘  要:目的探讨非瓣膜心房颤动患者血清miR-17、miR-145水平及其与白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、转化生长因子-β1(TGF-β1)、超敏C反应蛋白(hs-CRP)、血管内皮生长因子(VEGF)水平的关系。方法选取2020年9月至2022年3月在郑州市心血管病医院就诊的106例心房颤动患者作为试验组。选取同期在医院体检的60例健康志愿者作为对照组。采用反转录聚合酶链反应(RT-PCR)检测两组受试者血清miR-17、miR-145水平。采用酶联免疫吸附测定法检测血清IL-6、TNF-α、TGF-β1、hs-CRP和VEGF水平。分析心房颤动患者miR-17和miR-145水平与血清IL-6、TNF-α、TGF-β1、hs-CRP、VEGF的相关性。采用受试者工作特征(ROC)曲线分析miR-17、miR-145在心房颤动中的诊断价值。结果试验组患者血清miRNA-17水平高于对照组,miRNA-145水平低于对照组(P<0.05),IL-6、TNF-α、TGF-β1、hs-CRP、VEGF水平高于对照组(P<0.05)。患者血清miR-17水平与IL-6、TNF-α、TGF-β1、hs-CRP、VEGF水平呈正相关(P<0.05)。血清miR-145水平与IL-6、TNF-α、TGF-β1、hs-CRP、VEGF水平呈负相关(P<0.05)。血清miR-17、miRNA-145在心房颤动诊断中均具有较好的价值(P<0.05)。结论非瓣膜心房颤动患者血清miRNA-17表达上调,miRNA-145表达下调,与心房颤动的发生密切相关,对心房颤动有一定的诊断价值。Objective To explore the expression level of miR-17 and miR-145 in serum of patients with non-valvular atrial fibrillation,and their relationship with interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP),transforming growth factorβ1(TGF-β1)and vascular endothelial growth factor(VEGF)level.Methods A total of 106 patients with atrial fibrillation in Zhengzhou Cardiovascular Hospital from September 2020 to March 2022 were selected as the experimental group.And 60 healthy volunteers who were examined in hospital at the same time were selected as the control group.The expression levels of miR-17 and miR-145 in serum of the two groups were detected by reverse transcription-polymerase chain reaction(RT-PCR).The levels of IL-6,TNF-α,TGF-β1,hs-CRP and VEGF in serum were detected by enzyme-linked immunosorbent assay.The correlation between the expression levels of miR-17 and miR-145 in atrial fibrillation and the levels of IL-6,TNF-α,TGF-β1,hs-CRP and VEGF in serum was analyzed.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of miR-17 and miR-145 in atrial fibrillation.Results The level of miRNA-17 in the serum of patients in the experimental group was higher than that in the control group,and the level of miRNA-145 was lower than that in the control group(P<0.05),and the levels of IL-6,TNF-α,TGF-β1,hs-CRP and VEGF were higher than those in the control group(P<0.05).Serum miR-17 was positively correlated with IL-6,TNF-α,TGF-β1,hs-CRP and VEGF levels(P<0.05).Serum miR-145 was negatively correlated with IL-6,TNF-α,TGF-β1,hs-CRP and VEGF(P<0.05).Both miR-17 and miRNA-145 in serum had good values in the diagnosis of atrial fibrillation(P<0.05).Conclusion The expression of miRNA-17 is up-regulated and miRNA-145 is down-regulated in patients with non-valvular atrial fibrillation,which is closely related to the occurrence of atrial fibrillation and has certain diagnostic value for atrial fibrillation.
