基于微信平台的专人管理式家庭康复模式在高血压患者中的应用

摘　　要：目的探究基于微信平台的专人管理式家庭康复模式对高血压患者的影响,以期为临床提高慢性病管理水平提供指导。方法选取2021年1月至2022年1月河南省中西医结合医院收治的102例高血压患者,按照入院顺序分组,各51例。对照组接受常规干预,于此基础上观察组辅基于微信平台的专人管理式家庭康复模式,连续干预6个月。观察两组干预前后高血压治疗态度和信念、血压变化、慢性病管理评价(PACIC)及卫生服务利用情况。结果干预6个月后,观察组高血压治疗态度和信念量表各维度评分均高于对照组(P<0.05);干预3、6个月后观察组血压水平均低于对照组,舒张压变化值、收缩压变化值高于对照组(P<0.05);干预6个月后,观察组PACIC量表中各维度评分均高于对照组(P<0.05);6个月内,观察组复诊次数、再入院次数与对照组相比,差异无统计学意义(P>0.05)。结论基于微信平台的专人管理式家庭康复模式在高血压居家管理中取得显著效果,能协助其纠正治疗态度和信念,控制血压水平,减轻医疗资源使用压力,改善慢性病管理体验。Objective To investigate the influence of a dedicated management-based home rehabilitation model based on the WeChat platform on patients with hypertension,with a view to providing guidance for clinical improvement of chronic disease management.Methods One hundred and two patients with hypertension in Henan Integrative Medicine Hospital from January 2021 to January 2022 were selected and grouped according to the order of admission,with 51 cases in each.The control group received the conventional intervention and the observation group was supplemented by a dedicated management-based home rehabilitation model based on the WeChat platform for 6 months.Attitudes and beliefs about hypertension treatment,changes in blood pressure,evaluation of patient assessment of chronic illness care(PACIC)and utilization of health services were observed in both groups before and after the intervention.Results After 6 months of intervention,the scores of all dimensions of the attitude and beliefs about hypertension treatment scale were higher in the observation group than in the control group(P<0.05).The blood pressure levels in the observation group were lower than in the control group after 3 and 6 months of intervention,and the values of diastolic blood pressure change and systolic blood pressure change were higher than in the control group(P<0.05).After 6 months of intervention,the scores of all dimensions in the PACIC scale were higher in the observation group than in the control group(P<0.05).There was no statistical difference in the number of follow-up visits and readmissions in the observation group compared with the control group within 6 months(P>0.05).Conclusion The WeChat platform-based dedicated management-based home rehabilitation model has achieved significant results in the home management of hypertension,assisting in correcting their treatment attitudes and beliefs,controlling their blood pressure levels,reducing pressure on the use of healthcare resources and improving the chronic disease management experience.
