摘  要:目的探讨5A+5R护理模式联合饮食护理在老年溃疡性结肠炎中的应用效果。方法前瞻性选取2017年4月至2022年4月郑州大学第五附属医院收治的62例溃疡性结肠炎患者为研究对象,按随机数字表法将62例患者分为研究组31例、对照组31例。对照组接受饮食护理干预,研究组在对照组基础上接受5A+5R护理模式干预,两组患者均干预8周。比较两组各项心理状态评分[医院焦虑抑郁量表(HADS)、Mishel疾病不确定感量表(MUIS)、炎症性肠病自我效能量表(IBD-SES)]、营养状态[主观综合营养评估量表(SGA)]、营养指标水平[血清白蛋白(ALB)、总蛋白(TP)、血红蛋白(Hb)、前白蛋白(PAB)]。结果两组干预后HADS焦虑评分、HADS抑郁评分、MUIS评分与干预前相比均降低,IBD-SES评分(病症管理得分、压力与情绪管理得分、缓解期维持管理得分、医疗护理管理得分、IBD-SES总分)均升高(P<0.05);且研究组HADS焦虑评分、HADS抑郁评分、MUIS评分低于对照组,IBD-SES评分高于对照组(P<0.05)。两组干预后SGA等级的A级与干预前相比均改善,且研究组A级优于对照组(P<0.05)。两组干预后TP、PAB、ALB、Hb水平与干预前相比均升高,且研究组均高于对照组(P<0.05)。结论5A+5R护理模式联合饮食护理应用于老年溃疡性结肠炎患者可有效缓解负性情绪,提高自我效能感,改善机体营养状况,利于患者病情恢复。Objective To explore the application effect of 5A+5R nursing model combined with dietary nursing in elderly ulcerative colitis.Methods A total of 62 patients with ulcerative colitis who were treated in the Fifth Affiliated Hospital of Zhengzhou University from April 2017 to April 2022 were prospectively selected as the research objects.According to the random number table method,the 62 patients were divided into 31 cases in the study group and 31 cases in the control group.The control group was given diet nursing intervention,and the study group was given the 5A+5R nursing mode intervention on the basis of the control group.Both groups were given intervention for 8 weeks.The psychological state scores[hospital anxiety and depression scale(HADS),Mishel disease uncertainty scale(MUIS),inflammatory bowel disease self-efficacy scale(IBD-SES))],nutritional status[subjective comprehensive nutritional assessment scale(SGA)],nutritional index levels[serum albumin(ALB),total protein(TP),hemoglobin(Hb),prealbumin(PAB)]of the two groups were compared.Results After the intervention,the HADS anxiety scores,HADS depression scores and MUIS scores in the two groups were decreased compared with those before the intervention,and IBD-SES scores(symptoms management score,stress and emotion management score,maintenance management score during remission,medical care management score,IBD-SES total score)were higher than those before intervention(P<0.05).The HADS anxiety score,HADS depression score and MUIS score of the study group were lower than those of the control group,and the IBD-SES score was higher than that of the control group(P<0.05).The A of SGA grade in both groups after intervention were improved compared with those before intervention,and the research group A level was better than the control group(P<0.05).The levels of TP,PAB,ALB and Hb in the two groups after intervention were higher than those before intervention,and the study group were higher than the control group(P<0.05).Conclusion The application of 5A+5R nursing
