摘  要:目的 探讨白藜芦醇联合右美托咪定预处理对糖尿病大鼠急性肾损伤后的心肌保护效应。方法 健康雄性Wistar大鼠40只,建立2型糖尿病模型。将大鼠随机分为4组(n=8):对照组(Con组)、急性肾损伤组(AKI组)、右美托咪定预处理组(DPreC组)、白藜芦醇联合右美托咪定预处理组(Res+DPreC组)。采用切除右肾,左肾缺血45 min,再灌注4 h的方法建立AKI模型。以2μg·kg-1·h-1的速率于AKI前30 min起实施DPreC,AKI前15 min和再灌注前1 min注射白藜芦醇20 mg/kg。实验结束时留取心肌和血清标本。采用Western blot法测定心肌组织B细胞淋巴瘤-2(Bcl-2)、Bcl-2相关X蛋白(Bax)、沉默信息调节因子-1(SIRT1)、核因子-κB(NF-κBp65)蛋白表达及其磷酸化水平,采用ELISA法测定血清肌钙蛋白I(cTnI)、肿瘤坏死因子-α(TNF-α)、白介素-17a(IL-17a)、白介素-10(IL-10)水平,采用HE染色观察心肌病理学改变。结果 与Con组相比,AKI组和DPreC组p-NF-κB p65磷酸化和Bax蛋白表达上调,SIRT1、Bcl-2下调,cTnI、TNF-α、IL-17a升高,IL-10降低(P均<0.05),Res+DPreC组Bax上调,IL-10下降(P均<0.05);与AKI组相比,DPreC组、Res+DPreC组NF-κB p65磷酸化和Bax蛋白表达下调,SIRT1、Bcl-2上调,cTnI、TNF-α、IL-17a降低,IL-10升高(P均<0.05);与DPreC组相比,Res+DPreC组NF-κB p65磷酸化下调,Bcl-2蛋白上调,cTnI、TNF-α、IL-17a降低,IL-10升高(P均<0.05)。各组间NF-κB p65表达差异无统计学意义(P>0.05)。结论 白藜芦醇联合DPreC能够通过SIRT1-NF-κB通路缓解糖尿病大鼠急性肾损伤后心肌损伤。Objective To investigate the myocardial protective effect of resveratrol combined with dexmedetomidine preconditioning on acute renal injury in diabetic rats.Methods 40 healthy male Wistar rats were used to establish the model of type 2 diabetes.The rats were randomly divided into 4 groups(n=8):control group(Con group),acute kidney injury group(AKI group),dexmedetomidine preconditioning group(DPreC group)and resveratrol combined with dexmedetomidine preconditioning group(Res+DPreC group).AKI model was established by right kidney resection,left kidney ischemia for 45 min,and reperfusion for 4 h.DPreC was administered at a rate of 2μg·kg-1·h-130 min before AKI,and Res 20 mg/kg was injected 15 min before AKI and 1 min before reperfusion.At the end of the experiment,myocardial tissue and serum samples shall be taken.The expression of B-cell lymphoma-2(Bcl-2),Bcl-2 associated X protein(Bax),silent information regulatory factor-1(SIRT1),nuclear factor-κB(NF-κB p65)protein and p-NF-κB p65 in myocardial tissue was measured by Western blot.The serum levels of cardiac troponin I(cTnI),tumor necrosis factor-α(TNF-α),interleukin-17a(IL-17a)and interleukin-10(IL-10)were measured by ELISA.The myocardial pathological changes were observed by HE staining.Results Compared with the Con group,the phosphorylation of p-NF-κB p65 and the expression of Bax protein in the AKI group and DPreC group was up-regulated,SIRT1 and Bcl-2 was down-regulated,cTnI,TNF-α,IL-17a was increased,and IL-10 was decreased(P all<0.05).Compared with AKI group,p-NF-κB p65 and Bax protein expression was down-regulated,SIRT1 and Bcl-2 was up-regulated,cTnI,TNF-α,IL-17a was decreased,and IL-10 was increased in DPreC group and Res+DPreC group(P all<0.05).Compared with DPreC group,p-NF-κB p65 was down-regulatedand,Bcl-2 protein was up-regulated,cTnI,TNF-α,and IL-17a was decreased,and IL-10 was increased in the Res+DPreC group(P all<0.05).There was no significant difference in the expression of NF-κB p65 between the groups(P>0.05).Conclusion Res com
