揿针联合全息刮痧防治混合痔吻合器痔上黏膜环切术后尿潴留临床观察

摘　　要：目的观察揿针联合全息刮痧疗法防治混合痔吻合器痔上黏膜环切术(PPH)术后尿潴留的疗效。方法将120例混合痔PPH术后患者按照入院时间随机分为2组,治疗组60例予揿针联合全息刮痧治疗,对照组60例予膀胱区热敷联合诱导排尿法治疗。2组均治疗7天后统计疗效,比较2组术后首次排尿时间及首次排尿后膀胱残余尿量情况,比较2组术后第3、7天疼痛视觉模拟评分法(VAS)评分及焦虑自评量表(SAS)评分变化情况。结果治疗组术后首次排尿平均时间(170.82±59.57)min,对照组首次排尿平均时间(294.32±69.32)min,治疗组术后首次排尿时间短于对照组(P<0.05)。治疗组首次排尿后膀胱平均残余尿量(51.17±36.22)mL,对照组为(111.17±41.34)mL,治疗组首次排尿后膀胱残余尿量少于对照组(P<0.05)。治疗组术后第3、7天SAS评分分别为(2.82±2.29)、(1.72±1.81)分,对照组术后第3、7天疼痛VAS评分分别为(5.63±2.30)、(4.42±1.81)分,治疗组术后第3、7天疼痛VAS评分均低于对照组同期(P<0.05)。治疗组术后第3、7天SAS评分分别为(46.95±4.77)、(24.90±6.30)分,对照组术后第3、7天疼痛VAS评分分别为(49.03±6.58)、(43.00±7.53)分,治疗组术后第3、7天SAS评分均低于对照组同期(P<0.05)。治疗组总有效率93.3%(56/60),对照组总有效率68.3%(41/60),治疗组疗效优于对照组(P<0.05)。结论揿针联合全息刮痧防治混合痔PPH术后尿潴留疗效确切,可有效促进术后排尿,减少膀胱残余尿量,减轻疼痛症状,缓解患者焦虑情绪。Objective To observe curative effect of press-needle combined with holographic scraping on postoperative urinary retention in mixed hemorrhoid patients underwent procedure for prolapse and hemorrhoids(PPH).Methods Totally 120 mixed hemorrhoid patients undergoing PPH were assigned to the treatment group(n=60)and the control group(n=60)according to the admission time,in which they were treated with press-needle+holographic scraping and hot compress+induced urination,respectively.Treated for 7 days,the aim was to compare time to first postoperative urination,post-void residual bladder volume(PVRBV),visual analogue scale(VAS)for pain and self-rating anxiety scale(SAS)at 3 and 7 days postoperatively;the curative effect was assessed.Results The mean time to first postoperative urination and PVRBV in the treatment group were lower than those in the control group([170.82±59.57]min vs[294.32±69.32]min,[51.17±36.22]mL vs[111.17±41.34]mL,P<0.05).On days 3 and 7,VAS in the treatment group was lower than that in the concurrent control group([2.82±2.29]points vs[5.63±2.30]points,[1.72±1.81]points vs[4.42±1.81]points,P<0.05);SAS scores were lower in the treatment group than the concurrent control group([46.95±4.77]points vs[49.03±6.58]points,[24.90±6.30]points vs[43.00±7.53]points,P<0.05).The overall effective rate was higher in the treatment group(93.3%[56/60])compared with the control group(68.3%[41/60]),(P<0.05).Conclusion For mixed hemorrhoid patients underwent PPH,press-needle combined with holographic scraping has a definite curative effect on postoperative urinary retention,which can effectively promote postoperative urination,reduce residual bladder volume,relieve pain and anxiety.
