摘  要:目的:探究不同剂量肺表面活性物质联合经鼻无创高频通气治疗早产儿呼吸窘迫综合征的临床效果。方法:收集2019年1月-2022年1月本院收治的121例呼吸窘迫综合征患儿资料,根据药物剂量的不同分为高剂量组(n=62)、低剂量组(n=59),分别给予100 mg/kg、70 mg/kg牛肺表面活性物质联合经鼻无创高频通气治疗,对比两组住院时间、上机时间、临床疗效、血气分析指标、肺氧合功能[动脉血氧分压与肺泡氧分压的比率(a/APO_(2))、氧合指数(OI)]以及并发症。结果:高剂量组住院时间(10.3±2.3d)、上机时间(81.7±14.3h)低于低剂量组(13.9±2.2d、125.2±17.9h),临床总有效率(95.2%)高于对照组(81.4%)(均P<0.05),pH水平两组无差异(P>0.05),a/APO_(2)、OI水平高剂量组变化优于低剂量组(P<0.05),并发症发生率(8.1%)与低剂量组(13.6%)无差异(P>0.05)。结论:100 mg/kg肺表面活性物质联合经鼻无创高频通气治疗临床疗效提高,可更好有效改善患者血气相关指标以及肺氧合功能,且安全性较好。Objective:To explore the clinical effect of different doses of pulmonary surfactant combined with nasal non-invasive high-frequency ventilation for treating preterm infants with respiratory distress syndrome(RDS).Methods:The data of 121 preterm infants with RDS from January 2019 to January 2022 were collected.According to the different drug doses,these infants were divided into two groups.62 infants in the study group were given treatment of high-dose pulmonary surfactant(100 mg/kg)combined with nasal non invasive high-frequency ventilation and 59 infants in the control group were given treatment of low-dose pulmonary surfactant(70 mg/kg)combined with nasal non-invasive high-frequency ventilation.The hospitalization time,the time of respirator used,the clinical efficacy,the values of blood gas analysis indexes,and the pulmonary oxygenation function indexes,such as ratio of arterial oxygen partial pressure to alveolar oxygen partial pressure(a/APO_(2))and oxygenation index(OD),and the complications rate of the infants were compared between the two groups.Results:The duration of hospital stay(10.3±2.3d)and the time of respirator used(81.7±14.3h)of the infants in the study group were significantly lower than those(13.9±2.2d and 125.2±17.9h)of the infants in the control group.The total effective rate(95.2%)of the infants in the study group was significantly higher than that(81.4%)of the infants in the control group(all P<0.05).There was no significant difference in the pH value of the infants between the two groups(P>0.05).The changes of a/APO_(2) and OI levels of the infants in the study group were significantly better than those of the infants in the control group(P>0.05).There was no significant difference in the complication rate(8.1%vs.13.6%)of the infants between the two groups(P>0.05).Conclusion:100 mg/kg pulmonary surfactant combined with nasal non-invasive high-frequency ventilation for treating preterm infants with RDS can increase the clinical effect and can improve the blood gas-related indexes and pu
