摘  要:目的观察单味黄芩煎剂对内毒素“二次打击”脓毒症大鼠体内纤维结合蛋白(Fn)及肺组织血管内皮细胞蛋白C受体(EPCR)、腺苷酸活化蛋白激酶(AMPK)表达的影响。方法28只健康SPF级成年雄性SD大鼠,随机分为假手术组、模型组、黄芩正常剂量组、黄芩高剂量组,每组7只,予腹腔注射内毒素(LPS)(1 mg/kg),16 h后气管内注入LPS(5 mg/kg),建立大鼠脓毒症急性肺损伤模型。黄芩正常剂量、高剂量组建模术前6 h分别经口灌胃2 mL不同剂量的黄芩煎剂,其中正常剂量组为1 g/kg,高剂量组为4 g/kg;假手术组和模型组均注射等体积的生理盐水,每天给药1次,连续3 d。结束实验后留取肺组织、血标本,HE染色观察肺组织病理学变化,测定肺组织湿/干重比,酶联免疫吸附法检测血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、Fn,血气分析检测氧气分压(PaO_(2))并计算氧合指数,蛋白免疫印迹法(Western blotting)检测肺组织EPCR、AMPK。结果模型组较假手术组肺组织出现炎性病理改变,血清Fn明显下降,TNF-α、IL-6显著上升,湿干重比升高明显,氧合指数显著下降,肺组织EPCR表达降低明显,AMPK表达增加明显;与模型组相比,黄芩正常剂量组、高剂量组血清Fn明显升高,肺组织EPCR表达升高明显,湿干重比明显下降,氧合指数明显改善,减轻肺组织病理变化(均P<0.05或P<0.01);黄芩正常剂量组、高剂量组之间Fn、EPCR、AMPK表达差异无统计学意义(P>0.05),湿干重比、氧合指数差异无统计学意义(P>0.05)。结论单味黄芩煎剂对内毒素“二次打击”脓毒症大鼠肺损伤有保护作用,能够降低肺损伤程度,改善氧合指数,其机制可能与抑制炎症反应,提高Fn、EPCR表达,保护内皮细胞有关,但与黄芩的剂量关系不密切。Objective:To investigate the effect of single Huangqin Decoction on Fn in vivo and the expression of EPCR and AMPK in the lung tissue of rats with endotoxin"two-hit"sepsis.Methods:A total of 28 healthy adult male SD rats of SPF grade were randomly divided into the control group,model group,normal dose and high dose groups of single Huangqin Decoction,with 7 rats in each group.In the model group,LPS(1 mg/kg)was injected intraperitoneally,and LPS(5 mg/kg)was injected intratracheally 16 hours later to establish an acute lung injury model of sepsis in rats.In the normal dose group and high-dose group,2 mL of single Huangqin Decoction in different doses(1 g/kg,4 g/kg)were administered orally 6 hours before operation,respectively.In the sham operation group and model group,the same volume of normal saline was injected once a day for 3 consecutive days.After the experiment,the lung tissue and blood samples were taken.HE staining was used to observe the pathological changes of lung tissue and record the lung tissue injury score.The wet/dry weight ratio of lung tissue was measured.Serum TNF-a,IL-6 and Fn were detected by enzyme-linked immunosorbent assay.Partial pressure of oxygen(Pa()2)was detected by blood gas analysis and oxygenation index was calculated.EPCR and AMPK of the lung tissue were detected by Western blotting.Results:Compared with the control group,the model group displayed inflammatory pathological changes in the lung tissue,decreased serum Fn and EPCR expression of lung tissue significantly,increased TNF-a,IL-6 and AMPK expression of lung tissue significantly,elevated wet dry weight ratio increased significandy,oxygenation index decreased significandy(P<0.01 or P<0.05);compared with the model group,the serum Fn in the normal dose group and high dose group of single Huangqin Decoction increased significantly,the expression of EPCR in lung tissue increased significantly,the wet dry weight ratio decreased significantly,the oxygenation index improved significantly,and the pathological changes in lung tissue we
