动静结合针刺法辅助治疗后半规管良性阵发性位置性眩晕残余症状的疗效观察

摘　　要：目的观察“动静结合”针刺法辅助治疗后半规管良性阵发性位置性眩晕(PC-BPPV)复位后残余症状的临床疗效。方法将60例患者随机分为观察组与对照组各30例。对照组口服甲磺酸倍他司汀片,观察组在对照组的基础上加用“动静结合”针刺法,两组均治疗2个疗程。比较两组在治疗前、治疗1个疗程后、治疗结束后的眩晕障碍量表(DHI)评分、视觉模拟(VAS)评分;治疗完成后评估临床疗效;第6个月时进行电话随访,比较两组的复发率。结果两组患者DHI、VAS评分较治疗前降低(P<0.05)、治疗2个疗程后较治疗1个疗程后降低(P<0.05);治疗1个疗程、2个疗程后观察组DHI、VAS评分低于对照组(P<0.05);治疗结束后观察组总有效率高于对照组(P<0.05),观察组6个月内复发率低于对照组(P<0.05)。结论PC-BPPV患者复位后伴随残余症状者,在口服甲磺酸倍他司汀片的基础上,加用“动静结合”针刺法,可促进前庭功能恢复、改善残余症状、提高生活质量和降低复发率,且“动静结合”针刺法为辅助治疗的方法起效更快。Objective:To observe the clinical effect of"combination of dynamic and static acupuncture"on residual symptoms after reduction of posterior semicircular canal benign paroxysmal positional vertigo(PC-BPPV).Methods:A total of 60 patients with PC-BPPV who met the inclusion criteria and still had residual symptoms after repositioning were randomly divided into an observation group(30 cases)and a control group(30 cases).The control group was treated with betahistine mesylate tablets,and the observation group was treated with "combination of dynamic and static acupuncture"on the basis of the control group.Both groups were treated for 2 courses.The Dizziness Handicap inventory(DHI)score and Visual Analog Scale(VAS)score were compared between the two groups before treatment,after 1 course of treatment and after the end of treatment.The clinical efficacy was evaluated after treatment.Telephone follow-up was performed at the 6th month to compare the recurrence rate between the two groups.Results:There was no statistical difference between the DHI and VAS scores of the two groups before treatment(P>0.05);the DHI and VAS scores of the two groups were lower than before treatment(P<0.05)and after 2 courses of treatment than after 1 course of treatment(P<0.05);the DHI and VAS scores of the observation group were lower than those of the control group after 1 course and 2 courses of treatment(P<0.05);after the end of treatment The total effective rate of the observation group was higher than that of the control group(P<0.05),and the recurrence rate of the observation group within 6 months was lower than that of the control group(P<0.05).Conclusion:For PC-BPPV patients with residual symptoms after repositioning,on the basis of oral betamethine mesylate tablets,the"combination of dynamic and static acupuncture"can promote the recovery of vestibular function,improve the residual symptoms,improve the quality of life and reduce the recurrence rate.The combination of dynamic and static acupuncture is an auxiliary treatment method,which
