摘 要:目的探讨高通量血液透析(HFHD)对患者心功能和营养状况及炎症指标的影响。方法选取2016年6月至2021年5月收治的维持性血液透析(MHD)患者120例,随机分为两组各60例,对照组采用低通量血液透析(LFHD),观察组采用高通量血液透析(HFHD),比较两组治疗前及治疗后6个月肾功能指标、心功能指标、营养指标以及炎症指标。结果两组治疗后血尿素氮、肌酐、24 h尿蛋白定量均比治疗前明显降低,肌酐清除率比治疗前明显升高(P<0.05),但组间差异无统计学意义(P>0.05);治疗后观察组心率比治疗前降低,且低于对照组(P<0.05);治疗后观察组血红蛋白、血清白蛋白比治疗前升高,且高于对照组(P<0.05);治疗后观察组白介素-6、肿瘤坏死因子-ɑ、超敏C反应蛋白比治疗前降低,且低于对照组(P<0.05)。结论HFHD能够有效改善MHD患者心功能、营养状态及微炎症状态,是较理想的透析方式。Objective To investigate the effects of high-throughput hemodialysis(HFHD)on cardiac functional,nutritional status and inflammatory markers in patients.Methods A total of 120 patients treated with maintenance hemodialysis(MHD)in our hospital from Jun 2016 to May 2021 were selected and assigned into 2 groups randomly,the control group was treated with low-flux hemodialysis(LFHD),the observation group was treated with high-flux hemodialysis(HFHD),Renal function indexes,cardiac function indexes,nutritional indexes and inflammation indexes were compared between the two groups before treatment and 6 months after treatments.Results After treatment,the serum urea nitrogen,creatinine,24 h urine protein quantity was significantly lower than that before treatment,and the creatinine clearance rate was significantly higher in both groups(P<0.05),But there was no statistical significance between groups(P>0.05);After treatment,the heart rate of observation group was lower than that before treatment,and lower than that of control group(P<0.05);After treatment,the hemoglobin and serum albumin of the observation group was higher than those before treatment,and higher than those of the control group(P<0.05);After treatment,interleukin-6,tumor necrosis factor-alpha,high-sensitivity C-reactive protein of observation group was lower than those of before treatment,and lower than those of the control group(P<0.05).Conclusion HFHD is an ideal dialysis method,which can effectively improve the cardiac functional,nutritional status,and micro-inflammatory status in MHD patients.
