摘  要:经过40余年的发展,人工辅助生殖技术得到了广泛的应用,为数以百万计的家庭带来了福音。然而这些技术仍然存在许多挑战,如相对较低的妊娠成功率及体外受精-胚胎移植技术所生婴儿的健康风险高于自然受孕婴儿等。因此,需要进一步分析相关生理基础和技术程序。接受辅助生殖的女性妊娠失败主要发生在妊娠早期,对胚胎着床和子宫蜕膜化两大关键事件机制的探索就显得尤为重要。人子宫蜕膜化涉及到子宫内膜间质细胞剧烈的形态与功能的分化,对于妊娠成功至关重要。对人类而言,cAMP和孕激素信号是启动子宫蜕膜化的关键。文章总结了近年来蜕膜化中几个重大事件的研究结果,包括蜕膜化的启动、内膜基质细胞的改变、子宫腺分泌的增加及子宫自然杀伤细胞的增多,以期为进一步理解人类子宫蜕膜化内在机制与诸多不育问题及提高人工生殖技术的成功率提供参考。After more than 40 years of development,assisted reproductive technology has been widely used,bringing great hope to millions of families.However,many challenges remain with these technologies,including the relatively low success rate of pregnancy and the higher health risks of infants born with in vitro fertilization and embryo transfer technology than naturally conceived infants,which necessitate the further interpretation of associated physiological basis and technical procedures.Pregnancy failure in women undergoing assisted reproduction mainly occurs in the early stages of pregnancy,suggesting the significance of exploring the mechanisms of two key events,embryo implantation and decidualization.Decidualization of humane endometrium,which involves a significant morphological and functional differentiation of humane endometrial stromal cells(ESCs),is essential for the establishment of a successful pregnancy.For humans,cAMP and progesterone signaling are the keys to initiate the decidualization.This article summarizes recent studies about several major events in decidualization,including the initiation of decidualization,the changes of endometrial stromal cells,the increased secretion of uterine gland,the increased number of uterine natural killer cells.This review intended to provide a reference for deepening the understanding of the intricate mechanism of decidualization and many problems that are associated with infertility and improving the success rates of ART.
