摘 要:目的 探讨血清白介素-9(interleukin, IL-9)水平与环境等危险因素交互作用对青少年幽门螺杆菌(Helicobacter pylori,H.pylori)感染发生的影响,为H.pylori防控提供科学依据。方法 采用整体分层随机抽样的方法选取沧州市10所学校初中及高中年级学生526例进行^(13)C-尿素呼气试验,分析H.pylori感染情况及影响因素,并采用相加模型分析IL-9与H.pylori感染相关其他危险因素的交互作用。结果 H.pylori抗原阳性287例,感染率54.56%(287/526);Logistic回归分析显示,生活地区为农村、水源为井水或河水、生吃蔬菜、父母职业为工人或农民、父母有胃十二指肠疾病及血清IL-9水平低表达均为沧州市青少年H.pylori感染发生的独立危险因素(P<0.05);交互作用分析以干扰因素阴性(-/-)为对照,设OR值为1,IL-9降低与生活地区为农村、水源为井水或河水、生吃蔬菜、父母职业为工人或农民、父母有胃十二指肠疾病的交互作用指数分别为1.62、2.14、1.92、2.26和2.50。结论 生活地区为农村、水源为井水或河水、生吃蔬菜、父母职业为工人或农民、父母有胃十二指肠疾病及血清IL-9水平低表达均为沧州市青少年H.pylori感染发生的独立危险因素,且IL-9降低与其他危险因素间存在协同交互作用,IL-9应答可能在H.pylori抗感染免疫中发挥潜在的积极作用。Objective To investigate the effects of the interaction between interleukin-9(IL-9)and environmental and related risk factors on Helicobacter pylori(H.pylori)infection among adolescents,and provide a scientific basis for H.pylori prevention and control.Methods A total of 526 middle and high school students were selected from 10 schools in Cangzhouby stratified cluster sampling to undergo the ^(13)C-urea breath test.The status of H.pylori infection and its influencing factors were analyzed.An additive model was employed to analyze the interaction between IL-9 and H.pylori infection-related other risk factors.Results Among the 526 adolescents,287 were positive for H.pylori antigen,with an infection rate of 54.56%(287/526).The logistic regression analysis showed that living in rural areas,drinking water from wells or rivers,eating raw vegetables,parents being workers or farmers,parents having gastroduodenal diseases and a low serum IL-9 level were independent risk factors for H.pylori infection among adolescents in Cangzhou(P<0.05).With factors being negative(-/-)as the contrast and the corresponding odds ratio set to 1,the interaction analysis showed that the interaction index(S)values between a low IL-9 condition and living in rural areas,drinking water from wells or rivers,eating raw vegetables,parents being workers or farmers,and parents having gastroduodenal diseases were 1.62,2.14,1.92,2.26 and 2.50,respectively.Conclusion Living in rural area,drinking water from well water or river water,eating raw vegetables,parents as workers or farmers,parents with gastroduodenal disease and low serum IL-9 level are independent risk factors for H.pylori infection among adolescents in Cangzhou,and there is a synergistic interaction between IL-9 reduction and other risk factors.IL-9 may play a potential positive role in the anti-infection immunity of H.pylori.
