摘 要:目的分析老年人群健康生活方式评分与高血压、糖尿病和血脂异常发生风险的关系及其与冠心病的关联。方法对参与2016年武汉市某区社区健康体检的23288名老年人进行问卷调查、体格检查及实验室指标测定,根据吸烟、饮酒、饮食、运动、身体质量指数、腰围情况计算健康生活方式评分,采用SPSS 23.0进行多重线性回归和Logistic回归分析。结果与健康生活方式评分为0~1分的老年人相比,得分2、3、4、5~6者发生血脂异常的风险分别降低14%、25%、44%和52%。得分5~6者血清甘油三酯降低0.22 mmol/L,血清高密度脂蛋白胆固醇升高0.19 mmol/L,收缩压降低4.22 mmHg,舒张压下降2.45 mmHg。健康生活方式评分≥5分时,糖尿病及新发糖尿病发病风险较低,空腹血糖降低0.23 mmol/L。生活方式评分≥3分时,多病共患的发生风险显著降低。患有血脂异常和(或)高血压和(或)糖尿病的老年人发生冠心病的风险增加。多病共患的老年人发生冠心病的风险更高,当评分低于5分时此效应更为显著。结论健康的生活方式与较低的血脂异常、高血压和糖尿病发生风险有关,进而与低冠心病患病风险有关。Objective To explore the correlation between healthy lifestyle scores and the risk of hypertension,diabetes mellitus,and dyslipidemia,and the accompanying effect on coronary heart disease(CHD).Methods A total of 23288 elderly people who participated in a basic public healthy examination in a district of Wuhan City in 2016 were includoed in this study.Questionnaire survey,physical examination and blood sample collection and detection were conducted.The healthy lifestyle score was calculated based on smoking and drinking status,diet,exercise,body mass index(BMI),and waist.Multivariate linear regression and logistic regression analyses were performed by SPSS 23.0 to assess the associations between healthy lifestyle scores and risk of these diseases.Results Compared with the group scored 0-1,the risk for dyslipidemia were 14%,25%,44%and 52%lower in groups scored 2,3,4 and 5-6,respectively.The elderly scored 5-6 had a 0.22 mmol/L reduction in TG,a 0.19 mmol/L elevation in HDL C,a 4.22 mmHg reduction in systolic blood pressure and 2.45 mmHg reduction in diastolic blood pressure compared with the elderly scored 0-1.There was a negative correlation between lifestyle scores and risk of diabetes or new onset diabetes in population scored≥5,meanwhile there was a 0.23 mmol/L reduction in fasting glucose.The risk of comorbidity was also negatively correlated with lifestyle scores in population scored≥3.The risk for CHD was increased in the elderly with one or more diseases among dyslipidemia,hypertension,and diabetes.Older adults with comorbidities had a higher risk of coronary heart disease,especially among those scored less than 5.Conclusion Healthy lifestyle is related to lower risk of dyslipidemia,hypertension and diabetes,thereby it is associated with a lower risk of CHD.
