基于SMAS筋膜外区域性切除治疗耳前瘘管感染

摘　　要：探讨基于浅表肌腱膜系统(SMAS)膜外区域性切除术治疗耳前瘘管感染的疗效。选取耳前瘘管患者共126例,并分成3组:A组42例,耳前瘘管感染期,行SMAS筋膜外区域性切除术;B组43例,耳前瘘管非感染期但之前感染过,行SMAS筋膜外区域性切除术;C组41例,耳前瘘管感染期,行经典耳前组织切除术。统计所有患者术后的平均痊愈时间、切口外观满意度、总治愈率、复发率,并进行分析比较。结果显示,A组、B组的总治愈率相当,分别为95.24%和97.67%,差异无统计学意义(P>0.05);A组、B组的总治愈率均明显高于C组(65.85%),差异有统计学意义(P<0.05)。结果表明,感染期耳前瘘管应尽早通过手术彻底切除,基于SMAS筋膜外区域性切除术痊愈时间较短、总治愈率高、复发率低,无特殊设备要求,值得向各级医院推广。This paper investigates the effect of superficial muscular aponeurotic system(SMAS)extrafascial regional resection in the treatment of preauricular fistula infection.A total of 126 patients with preauricular fistula were selected and divided into 3 groups.In Group A,42 patients with preauricular fistula during the infection period,received SMAS extrafascial regional resection;in group B,43 patients with preauricular fistulas were treated with SMAS extrafascial resection;in group C,41 patients underwent classical preauricular tissue resection at the infection stage of preauricular fistula.The average recovery time,incision appearance satisfaction,total cure rate and recurrence rate of all patients were analyzed and compared.The results show that the difference between the total cure rate of the first two groups was not statistically significant(P>0.05)with 95.24%and 97.67%respectively.The total cure rate of the first two groups is significantly higher than that of Group C(65.85%),and the difference was statistically significant(P<0.05).It is suggested that the anterior auricular fistula in the infected period should be completely removed as soon as possible.Based on the short recovery time,high overall cure rate,low recurrence rate,and no special equipment requirements,SMAS extrafascial regional resection is worthy of promotion in hospitals at all levels.
