摘 要:目的分析子痫前期患者免疫功能变化及其与雌孕激素水平的相关性。方法选取500例孕妇,将符合子痫前期诊断的102例孕妇为子痫前期组(试验组),并按照病情严重程度分为轻度75例(试验1组)与重度27例(试验2组),剩余398例为正常孕妇组(对照组)。比较三组免疫功能指标变化情况,不同妊娠期雌孕激素水平及胎盘中雌二醇受体(E2R)及孕酮受体(PR)含量。结果三组CD8^(+)水平比较,差异无统计学意义(P>0.05);试验1组及试验2组CD4^(+)、辅助性T细胞17(Th17)高于对照组,调节性T细胞(Treg)水平低于对照组(P<0.05);且试验1组与试验2组比较,差异有统计学意义(P<0.05)。三组孕妇妊娠早期雌孕激素水平比较,差异无统计学意义(P>0.05);试验1组及试验2组孕妇在妊娠中期、后期的雌二醇及孕激素水平均低于对照组,其胎盘E_(2)R及PR含量均高于对照组(P<0.05),且试验1组与试验2组比较,差异有统计学意义(P<0.05)。子痫前期患者CD4^(+)、Th17与雌孕激素呈负相关,与E_(2)R、PR受体呈正相关(P<0.05);Treg与雌孕激素呈正相关,与E2R、PR受体呈负相关(P<0.05)。结论子痫前期患者免疫功能变化与雌孕激素水平存在相关性。Objective To analyze the changes of immune function in patients with preeclampsia and its correlation with estrogen and progesterone levels.Methods 500 pregnant women were selected,and 102 pregnant women who met the diagnosis of preeclampsia were classified as preeclampsia group(experimental group),and were divided into 75 cases of mild(test group 1)and 27 cases of severe(test group 2)according to the severity of the disease,and the remaining 398 cases were normal pregnant women group(control group).The changes of immune function indexes,estradiol receptor(E2R)and progesterone receptor(PR)contents in placenta were compared among the three groups.Results There was no significant difference in CD8^(+)level among the three groups(P>0,05).CD4^(+)and helper T cell 17(Th 17)in trial groups 1 and 2 were higher than those in control group,and regulatory T cell(Treg)levels were lower than those in control group(P<0.05).The difference between test group 1 and test group 2 was statistically significant(P<0.05).There was no significant difference in estrin levels among the three groups in early pregnancy(P>0.05).The levels of estradiol and progesterone in test group 1 and test group 2 during the second and third trimester were lower than those in the control group,and the contents of E2R and PR in the placenta were higher than those in the control group(P<0.05),and the difference between test group 1 and test group 2 was statistically significant(P<0.05).In preeclampsia patients,CD4^(+)and Thl7 were negatively correlated with eprogesterone,but positively correlated with E2R and PR receptors(P<0.05).Treg was positively correlated with estrogen,but negatively correlated with E2R and PR receptors(P<0.05).Conclusion There is a correlation between immune function and estrogen levels in preeclampsia.
