摘　　要：目的分析经阴道三维超声(3D-TVS)自由解剖成像诊断宫颈粘连的价值。方法回顾性分析2019年4月至2022年4月宜春市妇幼保健院收治的78例疑似宫颈粘连患者的临床资料,所有患者入院时均实施3D-TVS自由解剖成像检查、传统容积成像检查和宫腔镜检查,以宫腔镜检查结果为“金标准”,分析并比较3D-TVS自由解剖成像检查、传统容积成像检查诊断宫颈粘连的准确度、敏感度、特异度、阳性预测值、阴性预测值。结果3D-TVS自由解剖成像检查诊断宫颈粘连的准确度为97.44%(76/78),敏感度为97.77%(44/45),特异度为96.97%(32/33),阳性预测值为97.78%(44/45),阴性预测值为96.97%(32/33),Kappa值为0.947;传统容积成像检查诊断宫颈粘连的准确度为88.46%(69/78),敏感度为88.89%(40/45),特异度为87.88%(29/33),阳性预测值为90.91%(40/44),阴性预测值为85.29%(29/34),Kappa值为0.765。3D-TVS自由解剖成像检查诊断宫颈粘连的准确度高于传统容积成像检查诊断,差异有统计学意义(P<0.05);两种检查诊断宫颈粘连的敏感度、特异度、阳性预测值、阴性预测值比较,差异无统计学意义(P>0.05)。结论3D-TVS自由解剖成像检查诊断宫颈粘连的准确度较高,可为临床诊断提供依据。Objective To analyze the diagnostic value of transvaginal three-dimensional ultrasound(3D-TVS)free anatomical imaging in the diagnosis of cervical adhesions.Methods The clinical data of 78 patients with suspected cervical adhesions admitted to Yichun Maternal and Child Health Care Hospital from April 2019 to April 2022 were retrospectively analyzed.All patients underwent 3D-TVS free anatomical imaging examination,traditional volume imaging examination and hysteroscopy at admission.Taking hysteroscopy as the"gold standard",the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of 3D-TVS free anatomical imaging examination and traditional volume imaging examination in the diagnosis of cervical adhesions were analyzed and compared.Results The accuracy,sensitivity,specificity,positive predictive value,negative predictive value and Kappa value in the diagnosis of cervical adhesion were 97.44%(76/78),97.77%(44/45),96.97%(32/33),97.78%(44/45),96.97%(32/33)and 0.947 of 3D-TVS free anatomical imaging,and were 88.46%(69/78),88.89%(40/45),87.88%(29/33),90.91%(40/44),85.29%(29/34)and 0.765 of traditional volume imaging examination.The accuracy of 3D-TVS free anatomical imaging examination in the diagnosis of cervical adhesions was higher than that of traditional volume imaging examination(P<0.05).There were no statistical differences in the sensitivity,specificity,positive predictive value and negative predictive value between the two examinations(P>0.05).Conclusion 3D-TVS free anatomical imaging examination has high accuracy in diagnosing cervical adhesions,and can provide a basis for clinical diagnosis.
