摘　　要：目的探讨“一体两翼”慢病健康管理对社区中老年糖尿病患者生活质量、并发症发生率、糖化血红蛋白水平的效果。方法选取2020年4月至2021年4月湘潭市第三人民医院全科医学科收治的102例社区糖尿病患者作为研究对象,按照随机数字表法分为对照组(51例)与观察组(51例)。对照组采用常规护理干预方法,观察组采用“一体两翼”慢病健康管理方法。比较两组患者的生活质量、并发症发生率及糖化血红蛋白(HbA1c)水平。结果经12个月糖尿病干预管理后,观察组患者的并发症总发生率和HbA1c水平均低于对照组,生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论对社区中老年糖尿病患者实施“一体两翼”慢病健康管理后,患者的生活质量、并发症情况、糖化血红蛋白水平都得到明显改善。Objective To explore the role and effect of"one body and two wings"chronic disease health management on the quality of life,complication rate and glycosylated hemoglobin level of middle-aged and elderly diabetic patients in the community.Methods A total of 102 community diabetic patients admitted to the Department of General Medicine,Xiangtan Third People's Hospital from April 2020 to April 2021 were selected as the research subjects.According to random number table method,they were divided into the control group(51 cases)and the study group(51 cases).The control group adopted routine nursing intervention method,while the observation group adopted the"one body and two wings"chronic disease health management method.The quality of life,complication rate and glycosylated hemoglobin(HbA1c)level were compared between two groups.Results After 12 months of diabetes intervention management,the total complication rate and HbA1c level in the observation group were lower than those in the control group,and the life quality score was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion After implementing the"one body and two wings"chronic disease health management for middle-aged and elderly diabetic patients in the community,the quality of life,the incidence of complications,and the level of glycosylated hemoglobin have all been significantly improved.
