足月单胎臀位外倒转术成功的影响因素分析

摘　　要：目的:探讨足月单胎臀位外倒转术成功的影响因素。方法:选取2020年1月—2022年12月于南宁市红十字会医院产科定期产检的≥37孕周单胎臀位行外倒转术的275例孕产妇,对其妊娠结局进行分析,采用单因素分析及多因素Logistic分析探讨足月单胎臀位外倒转术成功的影响因素。结果:275例孕产妇进行外倒转术,成功132例(48.00%,无病例复变),失败143例(52.00%)。倒转成功的132例中,有16例剖宫产(12.12%);并发症有胎膜早破11例,胎心轻度变异减速16例,胎儿窘迫5例,无新生儿窒息。不同产次、胎盘位置、脐带异常及是否使用宫缩抑制剂的足月单胎臀位孕产妇的外倒转手术成功率比较,差异有统计学意义(P<0.05)。不同臀位类型、羊水量的足月单胎臀位孕产妇的外倒转手术成功率比较,差异无统计学意义(P>0.05)。多因素Logistic分析显示,经产妇、胎盘位置、脐带异常、使用宫缩抑制剂是影响外倒转手术成功的因素(P<0.05)。结论:经产妇、胎盘位置、脐带异常、使用宫缩抑制剂是影响足月臀位妊娠孕产妇臀位外倒转术成功的影响因素,临床中可针对这些因素给予相应的干预措施。Objective To explore the influencing factors of the success of the external buttock inversion in term single fetus.Methods From January 2020 to December 2022,275 pregnant women who underwent external breech reversal at more than 37 weeks’gestational weeks during regular obstetric examination in the obstetrics department of Nanning Red Cross Hospital were selected as the study subjects,and their pregnancy outcomes were analyzed,and univariate analysis and multivariate logistic analysis were used to explore the influencing factors of pregnancy outcomes.Results A total of 275 pregnant women underwent external buttock inversion,of which 132 cases(48.00%)succeeded and 143 cases(52.00%)failed.Complications included premature rupture of membranes in 11 cases,mild deceleration of fetal heart in 16 cases,fetal distress in 5 cases,and no neonatal asphyxia.There were statistically significant differences in the success rate of external buttock inversion in term single fetus pregnant women with different delivery times,placental location,umbilical cord abnormalities and whether to use uterine contraction inhibitors(P<0.05).There was no statistically significant difference in the success rate of external buttock inversion in term single fetus pregnant women with different breech position types and amniotic fluid volume(P>0.05).Multivariate logistic analysis showed that the factors affecting the success of external inversion surgery were multipara women,placental position,abnormal umbilical cord,and the use of uterine contraction inhibitors(P<0.05).Conclusion The factors affecting the success of external breech conversion in full-term breech pregnancy include multiparous women,placental location,abnormal umbilical cord,and the use of contraction inhibitors.Clinical intervention measures can be given for these factors.
