国内成人癌症患者鼻肠管肠内营养不良反应发生率的Meta分析

摘　　要：目的 系统评价国内成人癌症患者鼻肠管肠内营养的不良反应发生率。方法 计算机检索PubMed、Web of Science、中国生物医学文献数据库、维普数据库、中国知网、万方数据库,收集关于国内成人癌症患者鼻肠管肠内营养不良反应发生率的文献,检索时限为建库至2022年2月。两名研究者独立筛选文献,并分别提取资料、独立评价文献质量后,采用Stata 15.1进行Meta分析。结果 共纳入25篇文献,符合纳入标准的研究对象共2 132例。Meta分析结果显示,国内成人癌症患者鼻肠管肠内营养的不良反应发生率为41%[95%CI(33%~49%)]。亚组分析结果显示,发表年份为2005~2009年、胃癌、术后24 h后开始EN、EN开始前鼻饲温水、术前白蛋白水平≤35 g/L、住院时长<15天、置管深度距吻合口远端15~20 cm的成年癌症患者肠内营养不良反应发生率更高。结论 目前我国成人癌症患者鼻肠管肠内营养的不良反应发生率较高,应高度重视并积极采取预防措施。Objective To evaluate the incidence of adverse reactions of enteral nutrition support through nasoenteric nutrition tube in postoperative adult cancer patients in China.Methods PubMed,Web of Science,Chinese Biomedical Literature Database,VIP Database,China knowledge Network and Wanfang Database were searched by computer,and the literatures on the incidence of adverse reactions of enteral nutrition support through nasoenteric nutrition tube in domestic adult cancer patients were collected.The search time limit is from database establishment to 2022-02.The two researchers screened the literatures independently,extracted the data and evaluated the quality of the literatures independently,and then meta analysis was carried out by Stata15.1.Results A total of 25 articles were included,and 2132 subjects met the inclusion criteria.Meta analysis showed that the incidence of adverse reactions of enteral nutrition support through nasoenteric nutrition tube in domestic adult cancer patients was 41%[95%CI(33%~49%)].The results of subgroup analysis showed that the incidence of adverse reactions of EN support was higher in adult cancer patients with gastric cancer,published in 2005~2009,EN was initiated 24 hours after surgery,nasal feeding with warm water before EN,preoperative albumin level≤35 g/L,length of hospital stay<15 days,the end of the nasoenteric nutrition tube 15~20 cm away from the distal end of the anastomosis.Conclusion At present,the incidence of adverse reactions of enteral nutrition support through nasoenteric nutrition tube in domestic adult cancer patients after surgery is high,which should be paid more attention to and preventive measures should be taken actively.
