成人早期股骨头坏死诊断中应用CT和MRI检查的效果分析与探讨

摘　　要：目的:探讨成人早期股骨头坏死诊断中应用CT和MRI检查的效果。方法:选取2021年6月—2022年12月就诊于文安县中医院的80例早期股骨头坏死(femur head necrosis,FHN)患者(122髋)作为研究对象,所有患者均行计算机断层扫描(CT)和磁共振成像(MRI)检查。比较CT、MRI检查对FHN的诊断结果;分析CT、MRI诊断FHN的具体情况。结果:与CT检查(Kappa=0.592)比较,MRI检查对早期FHN的诊断效能(Kappa=0.872)更高;与CT对早期FHN分期准确率相比,MRI对早期FHN分期准确率更高,差异显著(P<0.05);MRI对早期FHN征象检出率高于CT,差异显著(P<0.05)。结论:CT和MRI检查对成人早期FHN均有着一定的诊断价值,然而相较于CT检查,MRI检查对早期FHN诊断效果更高,分组准确率更高,这对临床早期诊治有着重要的参考价值。Objective To discuss the diagnostic effect of CT and MRI in early adult femoral head necrosis.Methods 80 patients(122 hips)with avascular necrosis of the early femoral head necrosis(FHN)who were treated in Wen'an Hospital of Traditional Chinese Medicine from June 2021 to December 2022 were selected as the study subjects.All patients were examined by computed tomography(CT)and magnetic resonance imaging(MRI).Comparing the diagnostic results of CT and MRI for FHN,based on the surgical and pathological results as the diagnostic gold standard,the specific conditions of CT and MRI examination in the diagnosis of FHN were analyzed.Results Compared with CT(Kappa=0.592),MRI had a higher diagnostic efficiency for early FHN(Kappa=0.872);Compared with the accuracy of CT in early FHN staging,the accuracy of MRI in early FHN staging was higher,and the difference was significant(P<0.05);The detection rate of early FHN signs in MRI was higher than that in CT(P<0.05).Conclusion CT and MRI have certain diagnostic value for early adult FHN.However,compared with CT,MRI has higher diagnostic effect and grouping accuracy for early FHN,which has important reference value for clinical early diagnosis and treatment.
