彩色多普勒超声联合乳腺X线摄影在乳腺癌早期诊断中的临床价值

摘　　要：目的:探究早期乳腺癌应用彩色多普勒超声联合乳腺X线摄影的应用价值。方法:选取2021年5月—2022年10月于菏泽医专附属医院就诊的疑似乳腺癌患者88例,所有患者均行乳腺X线摄影及彩色多普勒超声检查。以手术病理结果为“金标准”,对比彩色多普勒超声、乳腺X线摄影及联合检查对不同体积早期乳腺癌的诊断准确率;对比单一及联合检查在早期乳腺癌中的诊断价值及一致性。结果:相比于各单项检查,联合检查在不同体积早期乳腺癌诊断中灵敏度、特异度及准确率均高于单一乳腺X线摄影及彩色多普勒超声检查,差异有统计学意义(P<0.05);乳腺X线摄影与彩色多普勒超声检查在不同体积的早期乳腺癌中的诊断准确率对比,差异均无统计学意义(P>0.05)。联合检查与手术病理结果一致性良好(Kappa=0.875);乳腺X线摄影与手术病理结果一致性尚可(Kappa=0.481);彩色多普勒超声检查与手术病理结果一致性不佳(Kappa=0.216)。彩色多普勒超声在早期乳腺癌恶性病灶<0.1 cm中诊断准确率显著低于联合检查(P<0.01)。结论:早期乳腺癌应用彩色多普勒超声联合乳腺X线摄影临床应用价值较高,能够明确病灶大小,且与“金标准”诊断一致性较强,具有较高的临床推广价值。Objective To explore the application value of color Doppler ultrasound combined with mammography in early breast cancer.Methods A total of 88 patients with suspected breast cancer admitted to the affiliated Hospital of Heze Medical College from May 2021 to October 2022 were selected.All patients underwent mammography and color Doppler ultrasonography.With the pathological results of surgery as the"gold standard",the diagnostic accuracy of color Doppler ultrasonography,mammography and combined examination for different volumes of early breast cancer was compared.To compare the diagnostic value and consistency of single and combined tests in early breast cancer.Results Compared with each single examination,the sensitivity,specificity and accuracy of combined examination in the diagnosis of early breast cancer in different volumes were higher than those of single breast mammography and color Doppler ultrasound,and the difference was statistically significant(P<0.05).There were no statistical differences in the diagnostic accuracy and diagnostic value of mammography and color Doppler ultrasound in different volumes of early breast cancer(P>0.05).The consistency between combined examination and surgical pathology results was good(Kappa=0.875);The consistency between breast mammography and surgical pathological results was acceptable(Kappa=0.481);The consistency between color Doppler ultrasound and surgical pathological results was not consistent(Kappa=0.216).The diagnostic accuracy of color Doppler ultrasound in malignant lesions<0.1cm of early breast cancer was significantly lower than that of combined examination(P<0.01).Conclusion The application of color Doppler ultrasound combined with mammography in early breast cancer has high clinical application value,can determine the size of the lesion,and has strong consistency with the"gold standard"diagnosis,which has high clinical promotion value.
