摘  要:目的:探讨40岁以下青年乳腺癌多模态超声表现与病理特征及免疫组织化学指标的相关性。方法:选取2018年4月—2022年6月于海南医学院第二附属医院进行术前超声检查的43例患者资料进行分析,观察患者的多模态超声图像特征,并分析其与病理特征及免疫组织化学指标的相关性。结果:青年女性乳腺癌病理类型多是中高级别的非特殊性浸润性癌,分子分型以Luminal型最为常见,免疫组织化学指标分子分型的病理类型、二维超声、彩色多普勒血流成像(color Doppler flow imaging,CDFI)及系统弹性评分差异无统计学意义(P>0.05);不同分子分型乳腺癌超声造影模式在增强强度、增强后病灶边缘、增强后是否灌注缺损方面,差异有统计学意义(P<0.05)。结论:多模态超声对40岁以下青年乳腺癌的诊断具有重要价值,其超声造影特征与分子亚型有一定关系,可为术前预测乳腺癌分子分型提供有价值的参考。Objective To investigate the relationship between multimodal ultrasound and pathological features and immunohistochemical indexes of breast carcinoma in young women under 40 years old.Methods The data of 43 patients who underwent preoperative ultrasound examination at the Second Affiliated Hospital of Hainan Medical University from April 2018 to June 2022 were analyzed to observe the characteristics of multimodal ultrasound images of the patients and analyze their correlation with pathological characteristics and immunohistochemical indicators.Results The pathological types of breast cancer in young women were mostly intermediate and high-grade non-specific invasive carcinoma,and the molecular typing was the most common luminal type,and there were no significant differences in the pathological type,two-dimensional ultrasound,color Doppler flow imaging(CDFI)and system elasticity score of immunohistochemical index molecular classification(P>0.05).There were significant differences in enhanced intensity,enhanced lesion margin,and whether there was perfusion defect after enhancement in different molecular types(P<0.05).Conclusion Multimodal ultrasound is of great value in the diagnosis of breast carcinoma in young patients under 40 years old,and CEUS features are related to molecular subtypes,which can provide valuable reference for predicting molecular typing of breast carcinoma before surgery.
