手足口病脑炎中的神经胶质和神经元标志物:S100B和NSE水平的Meta分析

摘　　要：目的:综合评价手足口病患者血液及脑脊液中S100钙结合蛋白β(S100B)及神经元特异性烯醇酶(NSE)的水平与发生脑炎的联系,为早期识别脑炎提供依据。方法:检索中国生物医学文献数据库、中国知网、PubMed、Embase等中英文数据库,检索年份为数据库建库至2022年9月。根据纳入与排除标准筛选文献,使用Stata软件对提取的数据进行Meta分析。结果:共纳入12篇文献,共计1089名手足口病患者,其中手足口病脑炎患者565人。结果显示,手足口病脑炎患者血液样本及脑脊液中S100B及NSE水平均较轻症患者更高:在血液中,S100B水平(SMD=2.44,95%CI:1.37-3.50),NSE水平(SMD=2.32,95%CI:1.67-2.97);在脑脊液中,S100B水平(SMD=4.88,95%CI:3.18-6.58),NSE水平(SMD=5.42,95%CI:3.36-7.48)。结论:发生脑炎的手足口病患者S100B及NSE水平明显升高,说明这两种生物标志物可能对脑炎的识别具有诊断作用。Objective:To comprehensively investigate on the association between the S100 calcium binding proteinβ(S100B)and neuron⁃specific enolase(NSE)levels in blood and cerebrospinal fluid of patients with hand,foot and mouth disease(HFMD)and the occurrence of HFMD⁃associated encephalitis,so as to provide evidence for early identification of this condition.Methods:Databases in Chinese and English languages,including the China Biomedical Literature Database,CNKI,PubMed and Embase,were searched from their inception up to September 2022.The hits were screened according to inclusion and exclusion criteria.A meta⁃analysis of the data extracted was performed using the Stata software.Results:A total of 12 articles were included with 1089 HFMD patients,of which,565 had HFMD⁃associated encephalitis.The results showed that the S100B and NSE levels in blood and cerebrospinal fluid samples of patients with HFMD⁃associated encephalitis were higher than those in patients with mild HFMD[blood levels:S100B(SMD=2.44,95%CI:1.37-3.50),NSE(SMD=2.32,95%CI:1.67-2.97);cerebrospinal fluid levels:S100B(SMD=4.88,95%CI:3.18-6.58),NSE(SMD=5.42,95%CI:3.36-7.48)].Conclusion:The levels of S100B and NSE are significantly elevated in HFMD patients complicated with encephalitis,indicating that these two biomarkers may have a diagnostic role in identification of HFMD⁃associated encephalitis.
