摘　　要：目的 探讨通络止痛汤治疗寒凝血瘀证盆腔炎性疾病后遗症的疗效。方法 将北京中医药大学房山医院妇科收治的90例寒凝血瘀型盆腔炎性疾病后遗症患者随机分成对照组和观察组各45例,对照组予妇科千金胶囊治疗,观察组在此基础上给予通络止痛汤治疗,2组均治疗1个月经周期。观察2组治疗前后中医症状积分、盆腔包块大小、盆腔积液及盆腔血流动力学指标变化,并比较2组总有效率和治疗方案的安全性。结果 观察组治疗后各项中医证候积分、盆腔包块大小、盆腔积液、子宫和卵巢的PI和RI均低于治疗前(P <0.05),且低于对照组(P <0.05),子宫PSV指标和卵巢PSV指标及治疗总有效率均高于对照组(P <0.05);对照组治疗前后子宫和卵巢的PI和RI指标无明显差异(P> 0.05);2组均无严重不良反应。结论 通络止痛汤能够调节寒凝血瘀型盆腔炎性疾病后遗症患者盆腔血流动力学指标,改善盆腔血瘀微环境,缓解患者寒凝血瘀络阻症状,安全可靠,疗效显著。Objective To explore the therapeutic effect of Tongluo Zhitong Decoction in the treatment of the sequelae of pelvic inflammatory disease with the syndrome of cold coagulation and blood stasis.Methods A total of 90 patients with the sequelae of pelvic inflammatory disease with the syndrome of cold coagulation and blood stasis admitted to the department of gynecology at the Fangshan Hospital of Beijing University of Chinese Medicine were randomly divided into a control group and an observation group,with 45 cases in each group.The control group was treated with the Fuke Qianjin Capsules,while the observation group was treated with the Tongluo Zhitong Decoction,both for one menstrual cycle.The changes in the TCM symptom scores,pelvic mass size,pelvic effusion and pelvic hemodynamic indexes were observed before and after treatment in the two groups,and the total eff ective rate and safety of the treatment plan were compared between the two groups.Results After treatment,the scores of TCM syndromes,pelvic mass size,pelvic effusion,PI and RI of uterus and ovary in the observation group were all lower than before treatment(P<0.05),and lower than those in the control group(P<0.05),while the uterine PSV index,ovarian PSV index and the total eff ective rate were higher than those in the control group(P<0.05).There was no significant difference in the PI and RI indexes of uterus and ovary in the control group before and after treatment(P>0.05).There were no serious adverse reactions in both groups.Conclusion The Tongluo Zhitong Decoction can regulate the pelvic hemodynamic indexes of patients with the sequelae of pelvic inflammatory disease of the cold coagulation and blood stasis pattern,improve the microenvironment of pelvic blood stasis,and relieve their symptoms of cold coagulation and blood stasis,which is safe,reliable and effective.
