摘  要:目的:腔内心电定位(IC-ECG)应用于经外周中心静脉置管(PICC)中的Meta分析。方法:搜索考伦图书馆、医学数据库、科学引文检索、中国知网、万方、维普数据库关于IC-ECG在PICC中的随机对照试验,采用一致性检验软件5.4统计分析结局指标。结果:最终纳入14篇文献,IC-ECG组和传统X线组PICC置管首次成功率分别为90.6%(n=2683),78.9%(n=1915),[OR=2.57,95%CI(2.16,3.06),P<0.001];精确度分别为78.6%(n=1008),68.6%(n=1015),[OR=2.7,95%CI(1.42,5.13),P=0.002];并发症发生率分别为4.2%(n=520),18.1%(n=518),[OR=0.18,95%CI(0.11,0.30),P<0.001],以上两组结局指标比较差异有统计学意义。结论:IC-ECG技术能提高PICC置管首次成功率及精确度、降低并发症发生率。Objective:To analyze the application of IC-ECG in peripheral central venous catheterization(PICC).Methods:Randomized controlled trials on IC-ECG in PICC were searched in Cochrane,PubMed,Web of science,CNKI,Wanfang and Pulse databases,and the outcome indexes were statistically analyzed by RevMan5.4 software.Results:Fourteen literatures were included.The first success rate of PICC catheter-placement in the IC-ECG group and the traditional X-ray group was 90.6%(n=2683)and 78.9%(n=1915),respectively[OR=2.57,95%CI(2.16,3.06),P<0.001].Accuracy was 78.6%(n=1008),68.6%(n=1015),(OR=2.7,95%CI(1.42,5.13),P=0.002).The complication rates were 4.2%(n=520)and 18.1%(n=518),respectively,[OR=0.18,95%CI(0.11,0.30),P<0.001].There were statistically significant differences in the outcome indexes between the two groups.Conclusion:IC-ECG technique can improve the first success rate and accuracy of PICC catheterization and reduce the incidence of complications.
