摘  要:目的:探讨脓毒症与血清代谢标志物之间的关系及可能的机制。方法:收集2019-03~2021-02就诊于漳州市医院的脓毒症患者,同期健康体检者为对照组。采用液相色谱-质谱联用技术(LC-MS)分析两组间血清代谢物含量的变化,应用正交偏最小二乘法(OPLS-DA)、ROC曲线筛选出两组中的差异代谢物,通过非靶向KEGG富集分析获得差异代谢通路。结果:该研究共纳入脓毒症患者20例,对照组20例,共筛选出17种重要差异代谢物,其中包含4个内源性代谢物:5’-甲硫腺苷,异亮氨酸丙氨酸,泼尼松龙,甾体。结论:5’-甲硫腺苷,异亮氨酸丙氨酸,泼尼松龙,甾体等血清代谢物可能与脓毒症的发病相关,其中5’-甲硫腺苷可能可以作为鉴别诊断早期脓毒症患者的临床实验室生物标志物。Objective:To explore the relationship and potential mechanism between sepsis and serum metabolomic biomarkers.Methods:Patients with sepsis admitted to Zhangzhou Hospital form Mar 2019 to Feb 2021 were collect.Healthy physical examination patients during the same period were selected as the control group.LC-MS was used to analyze the changes of serum metabolites between the two groups.The differential metabolites in the two groups were screened by orthogonal partial least squares(OPLS-DA)and ROC curve,and the differential metabolic pathways were obtained by non-targeted KEGG enrichment analysis.Results:A total of 20 patients with sepsis and 20 patients in the control group were included in this study.A total of 17 different metabolites were screened,including 4 endogenous metabolites:5'-adenosine methionine,isoleucine alanine,prednisolone,and steroid.Conclusion:5'-methionine,isoleucine alanine,prednisolone,steroids and other serum metabolites may be associated with the onset of sepsis,and 5'-methionine may be used as clinical laboratory biomarkers for the differential diagnosis of early sepsis patients.
