摘  要:目的探究晚期糖基化终末产物受体(RAGE)、白细胞介素-10(IL-10)基因多态性与社区获得性肺炎(CAP)并发脓毒症的关联。方法选取2019年12月-2021年5月浙江省安吉县第三人民医院就诊的114例CAP患者为研究对象,根据是否并发脓毒症分为合并脓毒症组51例和未合并组63例,比较两组基线资料及血清RAGE、IL-10水平,检测患者RAGE基因rs9469089、rs2070600、rs184003、rs2071288位点及IL-10基因A-1082G、A-592C位点多态性,比较各位点基因型及等位基因频率分布情况,分析RAGE、IL-10基因多态性与脓毒症易感性的关联。结果两组年龄、性别、合并症等基线资料比较,无统计学差异,合并脓毒症组急性生理与慢性健康评分(APACHE)Ⅱ、血清RAGE水平高于未合并组(P<0.05),血清IL-10水平低于未合并组(P<0.05);两组RAGE基因rs9469089、rs2070600、rs184003、rs2071288位点基因型及等位基因分布频率比较,无统计学差异,合并脓毒症组IL-10基因A-1082G、A-592C位点AA基因型、A等位基因分布频率高于未合并组(P<0.05);多因素Logistic回归分析显示,APACHEⅡ评分、血清RAGE、IL-10 A-1082G AA基因型及IL-10 A-592C AA基因型均为脓毒症易感性的相关因素(P<0.05)。结论IL-10 A-1082G、A-592C基因多态性与CAP患者并发脓毒症易感性有关。OBJECTIVE To explore the association between gene polymorphisms of receptor of advanced glycation end products(RAGE)and interleukin-10(IL-10)and sepsis in the patients with community-acquired pneumonia(CAP).METHODS A total of 114 patients with CAP who were treated in the Third People′s Hospital of Anji County from Dec 2019 to May 2021 were recruited as the research subjects and divided into the sepsis group with 51 cases and the no sepsis group with 63 cases according to the status of complication with sepsis.The baseline data and levels of serum RAGE and IL-10 were compared between the two groups.The polymorphisms at rs9469089,rs2070600,rs184003 and rs2071288 loci of RAGE gene and A-1082G,A-592C loci of IL-10 gene were detected,the frequencies of genotypes and alleles at the loci were compared,and the association of the gene polymorphisms of RAGE and IL-10 with the susceptibility to sepsis was observed.RESULTS There were no significant differences in the baseline data such as age,gender and complications between the two groups.The acute physiology and chronic health evaluation(APACHE)Ⅱscore and serum RAGE level of the sepsis group were higher than those of the no sepsis group(P<0.05),while the serum IL-10 level of the sepsis group was lower than that of the no sepsis group(P<0.05).There were no significant differences in the frequencies of genotypes and alleles at rs9469089,rs2070600,rs184003 and rs2071288 loci of RAGE gene between the two groups;the frequencies of AA genotype and allele A at A-1082G,A-592C loci of IL-10 gene were higher in the sepsis group than in the no sepsis group(P<0.05).Multivariate logistic regression analysis showed that APACHEⅡscore,serum RAGE,AA genotype at IL-10 A-1082G locus and AA genotype at IL-10 A-592C locus were the related factors for the susceptibility to sepsis(P<0.05).CONCLUSION The gene polymorphisms at IL-10 A-1082G and A-592C loci are associated with the susceptibility to sepsis.
