摘 要:目的探讨四肢骨折内固定术后切口感染血清转化生长因子-β1(TGF-β1)、碱性成纤维细胞生长因子(bFGF)表达与创面愈合的关系。方法选取2019年6月-2021年6月在武汉市中医医院行内固定手术治疗的65例四肢骨折术后发生切口感染患者为感染组,采集患者创面分泌物进行病原菌分离鉴定;另按倾向性匹配原则随机选取同期行相同手术后未发生切口感染的65例患者为未感染组;检测血清TGF-β1、bFGF、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,并比较创面愈合时间及创面愈合评分。采用Pearson相关性分析各指标与创面愈合时间及创面愈合评分的关系。结果65例切口感染患者创口分泌物中共检出病原菌68株,革兰阳性菌占比50.00%,革兰阴性菌占比45.59%;感染组患者血清TGF-β1、bFGF低于未感染组(P<0.05),IL-6、TNF-α水平高于未感染组(P<0.05),Pearson相关性结果显示,TGF-β1和bFGF均与IL-6、TNF-α呈负相关(P<0.05)。感染组患者创面愈合时间和愈合评分高于未感染组(P<0.05),Pearson相关性结果显示,TGF-β1、bFGF均与创面愈合时间、创面愈合评分呈负相关(P<0.05)。结论四肢骨折内固定术后切口感染患者血清TGF-β1、bFGF表达与切口愈合情况密切相关。OBJECTIVE To explore the relationship between expressions of serum transforming growth factor-β1(TGF-β1),basic fibroblast growth factor(bFGF)and wound healing of the limbs fracture patients with incision infection after internal fixation.METHODS A total of 65 limbs fracture patients who underwent internal fixation and had postoperative incision infection in Wuhan Hospital of Traditional Chinese Medicine from Jun 2019 to Jun 2021 were assigned as the control group,the wound secretions specimens were collected from the patients for culture of pathogens,the isolated pathogens were identified,meanwhile,65 patients who underwent the same surgeries but did not have postoperative incision infection were randomly chosen as the no infection group according to propensity matching principle.The levels of serum TGF-β1,bFGF,interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were detected,the wound healing time and wound healing score were compared between the two groups.Pearson correlation analysis was performed for the relationship between the indexes and the wound healing time as well as the wound healing score.RESULTS Totally 68 strains of pathogens were isolated from the wound secretions of the 65 patients with incision infection,of which 50.00%were gram-positive bacteria,and 45.59%were gram-negative bacteria.The levels of serum TGF-β1 and bFGF of the infection group were lower than those of the no infection group(P<0.05),the levels of IL-6 and TNF-αof the infection group were higher than those of the no infection group(P<0.05).Pearson correlation analysis showed that the TGF-β1 and bFGF were negatively correlated with the IL-6 and TNF-α(P<0.05).The wound healing time of healing score were higher in the infection group than in the no infection group(P<0.05);Pearson correlation analysis showed that the TGF-β1 and bFGF were negatively correlated with the wound healing time and wound healing score(P<0.05).CONCLUSION The expressions of serum TGF-β1 and bFGF in the limbs fracture patients with incision infection a
