VDR及MCP-1基因多态性与肥胖儿童反复呼吸道感染易感性的关联

摘　　要：目的分析维生素D受体(VDR)、单核细胞趋化蛋白-1(MCP-1)基因多态性与肥胖儿童反复呼吸道感染(RRI)易感性的关联。方法回顾性分析2018年6月-2021年7月温州医科大学附属第一医院158例肥胖RRI患儿临床资料为RRI组,并纳入同期入院体检的158名单纯性肥胖儿童为对照组,记录RRI组与对照组VDR基因位点rs7975232、rs731236及MCP-1基因位点rs1024611、rs3760399基因型及等位基因频率分布情况。结果RRI组与对照组VDR基因rs731236位点及MCP-1基因rs3760399位点基因型、等位基因频率比较,无统计学差异;RRI组VDR基因rs7975232位点CC/CA基因型频率及等位基因C频率均高于对照组(P<0.05),CC/CA型发生RRI的相对风险度OR值为1.790,携带等位基因C的OR值为1.600;RRI组MCP-1基因rs1024611位点GG基因型频率及等位基因G频率均高于对照组(P<0.05),GG基因型发生RRI的OR值为2.392,携带等位基因G的OR值为1.704。结论VDR rs7975232及MCP-1 rs1024611位点多态性与温州地区肥胖儿童RRI的易感性有关联,可为儿童RRI诊疗提供新方向。OBJECTIVE To observe the association between gene polymorphisms of vitamin D receptor(VDR)and monocyte chemoattractant protein-1(MCP-1)and susceptibility to recurrent respiratory infection(RRI)in children with obesity.METHODS A total of 158 obesity children with RRI who were treated in the First Affiliated Hospital of Wenzhou Medical University from Jun 2018 to Jul 2021 were enrolled in the study and assigned as the RRI group,the clinical data were retrospectively analyzed,meanwhile,158 children with simple obesity who received physical examination were chosen as the control group.The frequencies of genotypes and alleles at rs7975232,rs731236 loci of VDR gene and rs1024611,rs3760399 loci of MCP-1 gene were observed and compared between the RRI group and the control group.RESULTS There were no significant differences in the frequencies of genotypes and alleles at rs731236 locus of VDR gene and rs3760399 locus of MCP-1 gene between the RRI group and the control group.The frequencies of CC/CA genotype and allele C at rs7975235 locus of VDR gene were higher in the RRI group than in the control group(P<0.05).The OR value of RRI was 1.790 in patients carrying with CC/CA genotype,and the OR value was 1.600 in the patients carrying with allele C.The frequencies of GG genotype and allele G at rs1024611 locus of MCP gene were higher in the RRI group than in the control group(P<0.05);the OR value of RRI was 2.392 in the patients carrying with GG genotype,and the OR value was 1.704 in the patients carrying with allele G.CONCLUSION The gene polymorphisms of VDR rs7975232 and MCP-1 rs1024611 loci are associated with the susceptibility to RRI in the children with obesity,which provide new ideas for diagnosis and treatment of RRI.
