摘 要:目的 探究甘露糖结合凝集素(MBL)基因多态性及Toll样受体(TLR)2 mRNA、TLR4 mRNA表达与新生儿肺部感染的关联。方法 选取2020年6月-2021年6月武汉市妇幼保健院收治的肺部感染新生儿98例为病例组,随机抽取同期在医院分娩的健康新生儿100名为对照组。对两组MBL基因扩增后进行检测和分型,并采用荧光定量聚合酶链反应(PCR)测定两组TLR2 mRNA、TLR4 mRNA。比较两组MBL基因多态性及TLR2 mRNA、TLR4 mRNA表达差异。结果 病例组MBL基因-221位点G等位基因频率比对照组高(P<0.05);病例组MBL基因+4位点T等位基因频率比对照组高(P<0.05);病例组MBL外显子1基因+230位点G等位基因频率比对照组高(P<0.05);病例组MBL单体基因型中LYPB与LYQA基因频率比对照组高,LYPA基因频率比对照组低(P<0.05);TLR2 mRNA、TLR4 mRNA表达均比对照组高(P<0.05);经Logistic逐步回归分析显示,MBL基因-221位点G等位基因、+4位点T等位基因、+230位点G等位基因、LYPA单体基因、LYQA单体基因及TLR2 mRNA均为新生儿肺部感染的影响因素(P<0.05)。结论 MBL基因多态性与新生儿肺部感染存在一定相关性,TLR2 mRNA、TLR4 mRNA表达升高,临床可通过检测患儿MBL基因多态性及TLR2 mRNA、TLR4 mRNA表达评估肺部感染发生风险。OBJECTIVE To explore the association of mannose-binding lectin(MBL) gene polymorphisms, expressions of Toll-like receptor 2(TLR2) mRNA and Toll-like receptor 4(TLR4) mRNA with pulmonary infection in neonates. METHODS A total of 98 neonates with pulmonary infection who were treated in Wuhan Maternal and Child Health Hospital from Jun 2020 to Jun 2021 were assigned as the case group, meanwhile, 100 healthy neonates who were given birth in the hospital were randomly chosen as the control group. The MBL gene was amplified for the two groups of neonates, the genotypes of MBL were detected and identified, the expression levels of TLR2 mRNA and TLR4 mRNA were detected by means of fluorescent quantitative polymerase chain reaction(PCR), and the MBL gene polymorphisms and expression levels of TLR2 mRNA and TLR4 mRNA were observed and compared between the two groups. RESULTS The frequency of allele G at MBL gene-221 locus of the case group was higher than that of the control group(P<0.05);the frequency of allele T at MBL gene +4 locus of the case group was higher than that of the control group(P<0.05);the frequency of the allele G at MLB exon gene +230 locus of the case group was higher than that of the control group(P<0.05). The frequencies of LYPB and LYQA genotypes of MBL monosome gene of the case group was higher than that of the control group, while the frequency of LYPA genotype of the case group was lower than that of the control group(P<0.05). The expression levels of TLR2 mRNA and TLR4 mRNA of the case group were higher than those of the control group(P<0.05). Logistic stepwise regression analysis showed that the allele G at-221 locus of MBL gene, allele T at +4 locus, allele G at +230 locus, LYPA monosomic genotype, LYQA monosomic genotype and TLR2 mRNA were the influencing factors for the pulmonary infection in the neonates(P<0.05). CONCLUSION There is certain correlation between the MBL gene polymorphisms and the pulmonary infection in the neonates. The expression levels of TLR2 mRNA and TLR4 mRNA are elevated.
