摘  要:目的探讨月经性偏头痛(menstrual migraine,MM)患者发作间期不同脑区代谢特点。方法采用氢质子磁共振波谱成像技术检测36例MM患者和28例正常对照者(normal control,NC)内侧前额叶皮层(medial prefrontal cortex,mPFC)、丘脑的NAA、GABA、Glx、Cho、mI、Cr峰下面积,同时计算各代谢物与Cr的比值。结合独立样本t检验及Mann-Whitney U检验比较两组间差异,使用Spearman相关分析法探索代谢物指标与病程、发作频率和疼痛强度的相关性。结果1)代谢物比值比较:MM组左侧mPFC Cho/Cr值降低(P=0.003),余代谢物比值组间差异无统计学意义(P>0.05);双侧丘脑NAA/Cr、GABA/Cr、Glx/Cr、Cho/Cr、mI/Cr差异均无统计学意义(P>0.05);2)相关性分析:MM患者左侧mPFC Cho/Cr与发作频率呈负相关(r=-0.398,P=0.018),左侧丘脑Glx/Cr与疼痛强度呈正相关(r=0.347,P=0.020)。结论MM患者发作间期存在神经元功能障碍,其可能是偏头痛发作的易感因素。Objective To study the metabolic characteristics of different brain regions in patients with menstrual migraine(MM)during interictal period.Methods The subpeakal areas of NAA,GABA,Glx,Cho,mI and Cr in medial prefrontal cortex(mPFC)and thalamus of 36 MM patients and 29 normal controls(NC)were measured by hydrogen proton magnetic resonance spectroscopy.At the same time,the ratio of each metabolite to Cr was calculated.Independent sample t test and Mann-Whitney U test were used to compare the inter-group differences.Spearman correlation analysis methods was performed to explore the correlation between metabolites index and disease duration,attack frequency and pain intensity.Results 1)Metabolite ratio comparison:the Cho/Cr value of left medial prefrontal cortex in MM group was decreased(P=0.003),and there was no significant difference in other metabolite ratios between groups(P>0.05).Differences of NAA/Cr,GABA/Cr,Glx/Cr,Cho/Cr and mI/Cr between bilateral thalamus were not significantly different(P>0.05);2)Correlation analysis:In MM patients,the left mPFC Cho/Cr was negatively correlated with attack frequency(r=-0.398,P=0.018),and the left thalamic Glx/Cr was positively correlated with pain intensity(r=0.347,P=0.020).Conclusion There is neuronal dysfunction in MM patients during interictal period,suggesting that it may be a predisposing factor for migraine attack onset.
